Pivot Home Health is a new home health agency that is beginning to build a talented, compassionate and tenacious therapy team to provide physical therapy services to homebound patients in the Denver Metro Area. We are a locally owned agency with plans for quick, responsible growth, and ready to start hiring immediately.

Position Summary: The Physical Therapist is responsible for planning and implementing specific treatment programs for individual clients according to the principles and practices of physical therapy and according to the orders coordinated with a licensed physician.

Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Appropriately evaluates (initially and periodically throughout care) client in areas of range of motion/flexibility; postural alignment, muscle strength and endurance; sensation, reflexes, neurovascular status; soft tissue, bony palpation, passive joint mobilization; body mechanics and work efficiency; wound care; cardiopulmonary function; sitting and standing balance; mobility and gait training; and safety.
  • Develops and implements appropriate treatment programs to address problems identified; develops realistic personal goals with client; re-evaluates, sets new goals and adjusts treatment program accordingly.
  • Treats clients to develop or restore function and maintain maximum performance using physical means such as: therapeutic exercise, range of motion, coordination exercise, balance exercise, gait and transfer training with various weight bearing, prosthetic training, modalities, and others.
  • Advises, consults and instructs appropriate treatment procedures, therapeutic exercise and supportive activities to clients, family members and other home health team members.
  • Assesses need for adaptive and durable medical equipment. Instructs clients, family members and other home health team members in the care and use of the equipment. Develops needed plans for modifying equipment/appliances and physical surroundings in the home to enhance safety.
  • Observes, records and reports to physician and/or supervisor, client’s reaction to treatment, client’s progress and any changes in client’s condition.
  • Assists client and family with community resources needed to adjust to physical, mental and social limitations.
  • Coordinates other service team members, both in formal and informal care conferences, to assure sufficient flow of information to support care of client.
  • Informs client, family and physician verbally and in writing as needed regarding plan of care, progress, or changes and pending discharge.
  • Provides accurate and timely documentation of care in accordance with agency policy and pay source requirements.
  • Meets regulatory agency and/or pay source requirements regarding patient care.
  • Effectively manages a caseload as established by agency.
  • Participates, as requested, in orientation of new staff, in-service education, continuing education, utilization review and QI/QA programs.
  • Uses agency approved software to enter evaluations and visit documentation.

Abilities, Skills and Knowledge: To perform the job successfully, an individual should demonstrate the following.

  • Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and legal documents.
  • Ability to read and interpret physicians’ orders and notes from other caregivers as well as respond to them verbally and in writing.
  • Ability to effectively present information to clients and employees and community.
  • Ability to respond effectively to the most sensitive inquiries or complaints.
  • Ability to calculate figures and amounts such as area, circumference, and volume.
  • Ability to gauge distances.
  • Ability to measure and calculate angles using a goniometer.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to deal with several abstract and concrete variables. Ability to improvise in a home setting.
  • Ability to send/receive emails and use the internet for research and informational purposes.
  • Ability to use MS Excel to create a spreadsheet with minimal instruction.
  • Ability to use MS Word for communication and documentation purposes.
  • Ability to use independent judgment to manage and impart confidential information.
  • Skill to attend to multiple tasks at once which are time sensitive.
  • Knowledge of principles and scope of physical therapy.
  • Thorough knowledge of physical therapy theory and practice, home health care practice, communication skills, case management, and psychomotor skills.
  • Basic knowledge of Windows operating system and ability to advance computer skills as required.
  • Ability to establish and maintain effective working relationships with fellow employees, the public, supervisors and other County officials.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education/Experience: Bachelor's degree in Physical Therapy from an accredited school of Physical Therapy and two years of physical therapy experience. Home health experience is preferred.

Language Ability: Read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Write routine reports and correspondence. Speak effectively before groups of customers or employees.

Mathematical Ability: Add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Compute rate, ratio and percent, and draw and interpret bar graphs.

Reasoning Ability: Define problems, collect data, establish facts and draw valid conclusions. Interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Certificates, Licenses and Special Requirements:

  • Active Colorado Physical Therapist license
  • Meet all CMS requirements for education and experience
  • Current CPR certification
  • Current TB screening upon hire and annually thereafter
  • Possession of a valid state driver's license with reliable transportation and applicable car insurance if transporting patients
  • Proof of COVID-19 Vaccination or approved medical or religious exemption

Supervisory Responsibilities: Manages Physical Therapist Assistant employees. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

The employee is frequently exposed to outside weather conditions. The work environment is highly variable, as care is provided in private homes. Lighting, acoustics, air quality, temperature and environmental hazards are not predictable. The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals. The noise level in the work environment is usually moderate.

While performing the duties of this job, the employee is regularly required to sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with hands and arms and talk and hear. The employee is frequently required to use hands to finger, handle, or feel. The employee is occasionally required to climb and use sense of smell. The employee must be able to lift and/or move up to 15 pounds and occasionally lift and/or move 100 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus. The employee must have the ability to perform the anticipated level of physical assist required to provide safe client care. If the anticipated level of assist is greater than that which the employee can safely provide, the supervisor is to be notified immediately.

Travel: The employee is required to travel frequently to complete home health visits. There are occasional trips to locations outside the county.
