Description

POSITION SUMMARY

The function of a Therapeutic Mentoris to provide clients with the services and support necessary to assist the client in achieving and maintaining rehabilitative, resiliency and recovery goals. The Therapeutic Mentor facilitates skill building, identification and use of natural support, and use of community resources.

Essential Functions Statement

Complete individual treatment plans and associated assessments.

Meet or exceed billing standards while providing quality care to clients under Rule 132 guidelines.

Provide therapeutic mentoring services to clients on a weekly basis, based on the treatment plan.

Maintain contact with therapists 2-4 times monthly and program manager as needed.

Complete documentation in accordance with Rule 132 standards.

Attend required supervisions, meetings and trainings.

Work independently while scheduling appointments and meeting with clients/families.

Schedules meetings with clients and legal guardian (if applicable) to inform them of progress in treatment.

Develop and maintain healthy relationships with clients and their families.

Prepare accurate and timely records, understanding the importance and necessity for both client care and administrative responsibilities.

Performs all other duties as assigned by Supervisor.

Regular, reliable, predictable attendance.

Qualifications


SKILLS & ABILITIES

Education:

Meet MHP requirements per Rule 132: Bachelors degree in human services or related field, Bachelors degree in any other field with two years of supervised clinical experience, or High School diploma or GED and 5 years supervised clinical experience in mental health or human services

Experience:

1 plus years of experience in Job related field preferred

Computer Skills:

Evolv, Microsoft Office, Word, Excel, Powerpoint, Outlook

Certifications &
Licenses:

Must have and maintain a valid driver's license.

Other Requirements:

Must be able to meet agency driver safety requirement of carrying personal vehicle insurance with the following limits:
$50,000 bodily injury, each person
$100,000 bodily injury, each accident
$50,000 property damage

Ability to use Phone, Fax, Copier, and file required.
