Job Description

The Household/Neighborhood Clinical Leader(RN) is responsible for the 24-hour clinical care of the household’s residents and supervises all clinical team members in partnership with the Household/Neighborhood Leader. The Household/Neighborhood Clinical Leader II (RN) leads the care planning process for residents in their household/neighborhood and teaches and educates on clinical matters as needed in the community. May serve as a clinical mentor to other parts of the community in a specific area of expertise such as MDS, infection control, wound management, team member education or other area.

The Household/Neighborhood Clinical Leader(RN) shares leadership responsibilities with the Household/Neighborhood Leader and works cooperatively and respectfully with residents, the household/neighborhood team, support teams, families, visitors, and volunteers to create a positive and pleasant environment in the household/neighborhood; and takes part in in household/neighborhood activities with and for residents.

EDUCATION, EXPERIENCE, CERTIFICATION AND LICENSURE REQUIREMENTS 

  1. Graduate of an accredited school of nursing program with a diploma or a degree.
  2. Licensed as a Registered Nurse in the state of Virginia; maintains license in active status.
  3. Current CPR/AED certificate of completion required.
  4. ServeSafe certification.
  5. Four years of nursing experience working with senior population in a skilled health care facility or retirement facility.
  6. Two years of supervisory experience preferred.

Job Duties as Household/Neighborhood Clinical Leader 

  1. Resident Care: Plans, organizes, directs, coordinates, and evaluates nursing services for all shifts. Review documentation prepared by subordinates for completeness and accuracy daily. Ensures that significant changes in resident condition are communicated to the physician, family, or responsible party. Addresses concerns and complaints from residents, families, or physicians; refers unresolved issues to the Director of Nursing. Follows proper infection control practices; ensures that the infection control program is always maintained. Assists with call outs and communicates with on-call nursing management as needed to ensure adequate coverage.
  2. Clinical Rounds: Performs clinical rounds in the household/neighborhood daily for supervision of services, resident status, to assess resident safety and comfort, and for regulatory compliance. Holds clinical team members accountable for the nursing services they deliver to residents.
  3. Care Planning/MDS: Coordinate nursing practices toward restoring and maintaining a holistic approach to meet all residents’ physical, emotional, behavioral, psychological, familial, spiritual, social, and nutritional needs. Leads the care planning process for residents in their household/neighborhood including initial planning (upon admission), interim planning (clinical status changes), and the required documentation. Develops a plan of care with resident, household/neighborhood team members and family; updates care plans according to changes in resident condition or status. Monitor nursing documents daily. In a skilled nursing household/neighborhood, ensures that all sections of the comprehensive assessment tool (MDS) are complete and current.
  4. Admission, Transfer and Discharge: Oversees the resident admission into and discharge from the household/neighborhood and ensures that all steps have been completed; assists as needed to ensure efficiency and timely completion.
  5. Compliance: Takes part in in federal, state, and local surveys and inspections as requested; follows established plans of correction. Ensures that clinical services comply with federal, state, and local regulations and professional standards while honoring person-centered and resident-directed services. Keeps abreast of current state and federal regulations and professional standards and makes recommendations for incorporation into policies and procedures. Ensures that appropriate recordkeeping and documentation are maintained.
  6. Continuing Education: Attends in-service education programs and team conferences as scheduled; completes in-service programs as assigned.
  7. People Management: Along with the Household/Neighborhood Leader, helps recruit, select, orient, train and motivate household/neighborhood clinical team members to ensure professional standards of practice. Supervises all clinical household/neighborhood team members. Continuously assesses the clinical competencies of team members and addresses opportunities for improvement. Provides direction when needed without micromanaging.
  8. Team Leadership: With the Household/Neighborhood Leader, builds rapport among the household/neighborhood team. Encourages self-directed teams, team problem solving and conflict resolution. Partners with support teams. Leads by example.
  9. Operational Management: Keeps household/neighborhood clinical services running smoothly. Schedules clinical team members to best support the needs of the residents; works with Household/Neighborhood Leader on scheduling matters. Ensures that all needs are covered; minimizes overtimes and PRN expenses. Participates in on-call and weekend rotations.
  10. Performance Evaluation: Provides specific, frequent feedback to improve performance on job skills and household/neighborhood competencies; addresses individual performance problems timely; develops performance improvement plans for underperforming team members; celebrates individual and team successes. Completes performance evaluations of household/neighborhood clinical team members at least annually; gives input into performance evaluations of non-clinical household/neighborhood team members and support team members.

Qualifications

  1. Comprehensive knowledge of practices and procedures of support services; practices and procedures for clinical documentation.
  2. Advanced knowledge of applicable federal, state, and local laws and regulations.
  3. Management and supervisory principles and practices.
  4. Ability to meet the needs of residents using patience, tact, and a desire to serve seniors.
  5. Ability to read and write, follow instructions, and communicate effectively in English.
  6. Ability to assign, supervise and review the work of others.
  7. Advanced knowledge of computer applications and skills with an aptitude to learn company software.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

  1. Capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching, kneeling, reaching over the head, grasping with both hands, and fine manipulation.
  2. Physically be able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment.
  3. Physically be able to lift up to 50 pounds unassisted.
  4. Normal eyesight or use corrective lenses to achieve it.
  5. Ability to hear and distinguish between normal tones and be able to perceive the nature of sounds.
  6. Ability to move around and to work inside and outside to make rounds daily throughout the community and to respond to problems and emergencies.
