Job Summary:

Pinnacle Home Care is seeking Registered Nurses (RN) to join our Home Health Weekend Team- PRN! This individual contributor role will be responsible for providing home health services to patients in the Jacksonville, Florida area. The successful candidate will work Saturdays and Sundays and have the opportunity to pick up additional hours as needed (PRN).

Responsibilities:

- Perform initial and ongoing assessments of patients' medical, physical, and emotional needs and develop individualized care plans
- Provide skilled nursing care and treatments, including wound care, catheter care, and medication administration, in accordance with physician orders
- Educate patients and their families about their conditions, medications, and self-care techniques
- Monitor patients' progress and coordinate care with other healthcare professionals, including doctors, physical therapists, and social workers
- Document all care provided and communicate any changes in patients' conditions to the appropriate healthcare team members
- Adhere to all HIPAA, infection control, and OSHA standards
- Act as a liaison between patients, families, and the healthcare team
- Maintain professional and ethical standards at all times

Requirements:

- Current and unrestricted RN license in the state of Florida
- Minimum of 1 year nursing experience In Home Health, Including Oasis Documentation
- Knowledge of Medicare regulations and home health standards of care
- Excellent communication, time management, and critical thinking skills
- Ability to work independently and adapt to changing situations
- Valid driver's license and reliable transportation
- Ability to pass a background check and drug test

Pay: $32.00 - $36.00 per hour
