Responsibilities include:
- Reviewing case referrals and assigning appropriate clinicians
- Evaluating patient's clinical needs and assisting in goal-setting
- Attending case conference meetings and supporting new personnel
- Ensuring compliance with professional standards and Agency policies
- Providing ongoing education and support to clinicians
- Promoting patient and physician portals, as well as Telehealth visits
- RN with current licensure
- Graduation from an accredited school of nursing; BSN preferred
- Two years of management or supervisory experience in healthcare
- CPR certification
- Strong observation, organization, and communication skills
- Proficiency in computers, Microsoft, EMR, and technology
- Knowledge of governmental regulations and ACHC standards
