Summary

The Care Coordinator role is a performance-based health care sales position and is responsible for generating business and meeting targets from an assigned territory and interacting in the community to promote company services. Primary referral sources include hospitals, physicians, senior retirement communities, skilled nursing facilities, and the community-at-large. Responsible for identifying opportunities for formal or contractual relationships with physicians, hospitals, skilled nursing facilities and assisted living facilities, as well as executes territory sales plans in coordination with operational and clinical leadership to ensure extraordinary customer service.

Responsibilities

  • Adheres to the company's Mission and Values.
  • Analyzes potential prospects within specific market territory by reviewing past and current marketing data.
  • Understands knowledge and promotes services provided within market.
  • Develops relationships with prospective sources; hospitals, physicians, SNF's and ALF's to enhance sales growth. 
  • Creates weekly plans that reflect appropriate daily sales calls. 
  • Maintains accurate account list for all prospective referral sources and maintains physician profiles, weekly territory sales call plan and tracks all patient referrals daily thru start of care.
  • Meets and/or exceeds sales target for growth. 
  • Attends community strategic meetings within prospective territory. 
  • Develops focus driven marketing strategy with Sales Manager on a weekly basis. 
  • Maintains market awareness and communicates updates effectively. 
  • Organizes, distributes, and tracks respective patient documents for organization.
  • Communicates and assists operations as needed to ensure timely admissions, patient, and referral source satisfaction. 
  • Attends company sales/marketing meetings.
  • Adheres to company's HIPAA privacy, Business Ethics, Compliance programs
  • Adheres to Agency programs, policies, and procedures through: 
    • Awareness of the Agency’s QAPI program and active participation as required per policies and procedures related to the Agency’s QAPI program
    • Awareness of how the Agency manages and follows up on customer complaints, following the Agency’s complaint policy
    • Acute awareness of patient rights and the Agency’s responsibility in honoring patient's rights. Account Specialist will have an awareness of patient rights description in Agency's service agreement and Agency’s admission packets
    • Awareness of appropriate reasons for a patient discharge and Agency's policy on appropriate reasons for discharge
    • Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect
    • Awareness of infection prevention and control techniques within the Agency's policy and procedures and the Director of Clinical Service's oversight of this program, along with the QAPI designed program
    • Awareness of employee infection policies and procedures and the Director of Clinical Service’s oversight of the program, along with the QAPI designed program

Qualifications

Education/Experience

  • At least two years of medical sales experience WITH a current Book of Business
