We offer services directly in residents' homes, including preventative care, chronic condition management, and laboratory tests. Our primary care team visits each community regularly, working closely with staff to address individual health concerns. Additionally, we provide rapid access to medical care by offering same-day or next-day appointments for urgent issues, helping reduce the need for unnecessary emergency room visits and hospital admissions.
Our innovative model has attracted over 160 community partnerships and accelerated patient growth across California, Arizona, and Nevada. The Opportunity
Pine Park Health is looking for part time or full time Nurse Practitioners responsible for delivering care in our Marin County market. If you are excited about the opportunity to work with a smaller patient panel of complex care patients, contribute to a value-based care model, and work with a motivated team to provide high-quality patient care you have come to the right place. This person-centered care model works to leverage the power of our care navigators to maximize the time and energy of our providers.

Significant travel and startup hours are required for this role. You must have the ability to drive between partner communities as needed.
Pine Park Health provides the highest safety and quality of care for patients who can be at high-risk of COVID-19. To support this work, we require vaccination for all employees and proof of vaccination before hiring.
The salary range for full-time for this role is $140,000 to $185,000 inclusive of base, annual performance bonus, sign-on bonus, plus equity and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to, education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, organizational considerations, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
