Piedmont Fayette Hospital is Hiring a Care Management Assistant, Full Time Days, Piedmont Fayette Near Fayetteville, GA

JOB PURPOSE: As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.

KEY RESPONSIBILITIES:
1. Assists Care Managers and Social Workers in removing non-clinical barriers to discharge and securing post-acute care and community-based services from the Emergency Department and / or the Inpatient setting
2. Meets with patients, families and caregivers to identify and address non-clinical barriers to discharge, such as arranging follow up appointments and transportation.
3. Supports the post-acute referral process to external providers (i.e. Skilled Nursing Facility, Home Health, Rehabilitation, Hospice, Ambulatory Care, Durable Medical Equipment, other as needed / identified)
4. Utilizes electronic health record and other software applications to support role responsibilities
5. Scans documents into patient medical record, as needed to support CMS guidelines and discharge planning
6. Supports procurement of needed office supplies
7. Manages department mail
8. Maintains resource list and community resources (i.e. pamphlets and resource guides)
9. Participates in PHC readmission management initiatives
10. Follows protocol for reporting to legal agencies as needed with respect to abuse and neglect
11. Performs other tasks as assigned by the Care Manager or Social Worker

KNOWLEDGE, SKILLS, ABILITIES
Communication skills with focus on good customer service
Skill and ability to handle multiple priorities and deadlines
Ability to work as a member of a team
Skill and ability in Microsoft Office applications
Ability to take initiative and follow through
Knowledge of hospital audit and accreditation process by a hospital accreditation organization such
as Det Norske Veritas (DNV) or The Joint Commission

MINIMUM EDUCATION REQUIRED: High School diploma, or the equivalent such as the General Education Development (GED)
MINIMUM EXPERIENCE REQUIRED: Two (2) years of experience in a healthcare or social services setting
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: None
ADDITIONAL QUALIFICATIONS: Associate's degree or two years of college in human services/health related discipline preferred. Knowledge of community-based / post-acute care resources such as primary care, skilled nursing facilities, home health, durable medical equipment, and transportation.
