Job Description: The Practice Transformation Coach is responsible for building leadership and systems at physician practices to support sustainability in process and clinical outcomes. Essential Functions: Establishes and develops QI teams within practices Functions as a facilitator and change agent by utilizing outcome and quality data at provider practices Assesses and documents practice progression through identified tools and databases Summarizes and disseminates lessons learned by way of team updates, written reports/articles, and/or presentations Maintains an open dialog with practice team, peers, manager, and director Helps practices identify and problem solve operational and system barriers that will lead to redesigned office systems and processes Establishes clear goals and objectives with practice teams that align with client goals Facilitates practice-level QI Teams and manages improvement team meetings to become self-sufficient without coaching support Monitors practice performance including goals and objectives that align with initiatives Links practices to available internal and external resources to meet specific needs which extend beyond those that are met by the QI Coach’s role Educates provider practices on client specific goals and initiatives which are associated with clinical workflows and outcomes Assists the practices in maximizing available health information technology resources (e.g. electronic health records, disease registry, etc.) in order to accomplish improved patient outcomes and increased office efficiency Guides teams through improvement process & impart knowledge of tools & techniques to create sustainable systems, better clinical outcomes and greater patient satisfaction Presents data and results in a manner that is easy to understand and facilitates process improvement at all levels Adheres to the company’s Compliance Program and to federal and state laws and regulations Other duties as assigned Knowledge, Skills and Abilities: Knowledge of provider office workflow processes, healthcare delivery systems and healthcare trends Proven project management experience Ability to understand strategic direction of clients and apply to current position and practice support Excellent verbal communication, written communication, and presentation skills Great customer service skills Teamwork skills Independent problem-solving skills Self-motivated and self-managed Strong skills in Microsoft Office Suite Qualifications: Bachelor’s degree in Healthcare related field or equivalent experience in health care AND quality improvement Master’s in Public Health, Health Administration, Information Technology or related field preferred 3 - 5 years quality improvement experience with an established track record of leading successful process improvement initiatives, including implementing behavioral and system changes in a healthcare setting Knowledge of the Chronic Care Model and patient self-management principles, process improvement tools and techniques A valid unrestricted drivers’ license Home office that is HIPAA compliant for all remote or telecommuting positions as outlined by the company policies and procedures.
