The Program Monitor and Referral Coordinator is an intensive position with two main areas of responsibility: (1) the provision of monitoring and support to DHS Parenting Collaborative provider agencies conducting parenting education program. This responsibility represents approximately 90% of the full time position; and (2) manage the referral service to ensure that all requests for parenting education services are addressed and accurately documented (both communication and outcomes).

Program Monitor Responsibilities:

  • Establish relationships with agencies in order to provide guidance and support
  • Schedule, conduct, and document site visits to assess program capacity, services, quality and compliance
  • Maintain active working files for each agency which contains all reports and documentation, and provide copies of all documentation for master files
  • Assist agencies in complying with evaluation procedures as required by DHS
  • Monitor and document agencies’ performance, including successes and problems in meeting goals and complying with program standards and requirements
  • Assist agencies that are experiencing problems in developing plans to address problems and conduct regular follow-up to ensure plan is adhered to
  • Review data intake, monthly class cycle and invoicing data submissions to assess program compliance and activity
  • Work with Fiscal Compliance Analyst, and assist in following-up with agencies to assure they are invoicing appropriately
  • Work with programs to ensure that agencies are able to receive DHS referrals
  • Research and distribute information to agencies regarding best practices, innovative service models, and other information regarding the provision of parenting services
  • Participate in monthly team meetings and general meetings of Collaborative agencies
  • Provide monthly written and oral reports to supervisor on each agency
  • Assist in database support and problem-solving
  • Identify concerns/issues both specific to individual agencies and common to all grantees, discuss concerns with supervisor, and assist in development of plans to address concerns
  • Assist in continuation application and/or proposal review
  • Assist in addressing specific questions and concerns from DHS

Referral Coordinator Responsibilities:

  • Accept referrals from DHS, provider agencies and other sources of parents in need of parenting education programs
  • Complete standard assessment of parenting education needs, family demographics, referral source and other pertinent information
  • Make referrals to DHS Parenting Collaborative groups which best meet parents’ needs
  • Follow-up and confirm all referrals to assure that intended services are received
  • Document all referrals and confirmations in the DHS Parenting Collaborative Referral Services Access database
  • Provide referral data for reports to DHS
  • Work to ensure that providers are reporting back on the status of all referrals made, with the goal of minimizing the number of pending referral records
  • Ensure that providers are submitting up to date class cycle information to support the referral process and communicate with the Project Director when this is not the case
  • Make presentations about the DHS Parenting Collaborative to DHS, social service agencies, and other community organizations

Skills:

  • Extensive experience working with community-based organizations serving parents and families. Familiarity with policies and practices related to the delivery of parenting services
  • Proficient in Microsoft Excel, Word, and Access
  • Excellent written and oral communication skills
  • Strong organizational and analytical skills
  • Ability to assess organizational capacity and service delivery
  • Excellent administrative and organizational skills and attention to detail
  • Ability to prioritize multiple assignments
  • Ability to work both independently and as part of a team

Education and Experience:

  • Master’s degree required - preferably in social work or public policy and administration or related field
  • 10 or more years of experience in parenting education or other parenting support program or agency

Salary:

  • Commensurate with education and experience
  • Grade 18
