JOB DESCRIPTION

PHMC’s Forensic Services Department is looking for a full time Peer Recovery Specialist to provide peer recovery support to DCRP participants, within the Community Engagement Division. The Peer Recovery Specialists will assist the participant with their recovery, goals and will work in close collaboration with a team of individuals supporting program participants. This is an in-person, field-based position which will also include work within the Delaware County jail.

Responsibilities:

  • Complete weekly face to face meetings and daily phone contacts with participants to identify strengths and challenges to support their reentry into the community from incarceration.
  • Identify community linkages to support their recovery efforts which includes job readiness, educational, vocational, medical, mental, housing and family support.
  • Outreach to family members of program participants to assist with SDOH concerns which can serve as roadblocks to the participants successful reentry.
  • Provide support to participants to increase support reentry success and reduce recidivism.
  • Complete initial outreach (including visits to individuals while incarcerated) and orientation to the program.
  • Provide services in the County jail, home, treatment facilities, employment locations and other community-based sites to support participants.
  • Escort participants as needed on appointments for other social, behavioral or health care services as needed to enhance participant compliance with program requirements.
  • Provide education and support in conjunction with the DCRP Case Manager/Evaluator. Provide outreach to participants at treatment facilities, in the community.
  • Work as a team member with the DCRP Case Manager/Evaluator to ensure consistency with service provision and support to program participants.
  • Engage DCRP participants by sharing personal story of lived experience with the criminal justice system.
  • Attend reentry plan meetings with participant, probation, and community-based services providers reentry support.
  • Provide continuity of care as defined by reentry plans for assigned participants.
  • Assist participants in reentry in areas such as self-esteem enhancements, conflict resolution, assertiveness training, and other recovery skills.
  • Alert the DCRP Case Manager/Evaluator and Supervisor to any current or potential behavioral or health related problems of participants which may impact participant program compliance.
  • Provide comprehensive wellness support to participants which includes routine primary care and specialized health services as identified.
  • Document participant services into PHMC participant record in accordance with program protocols.
  • Provide on-call/after hours support on a rotational basis with other members of the PHMC Delaware County project team.
  • Ability to work a flexible schedule to meet the needs of program participants.

SKILLS: Good people skills

  • Knowledge of behavioral health and social services network in Delaware County
  • Lived experience with the criminal justice system.
  • Ability to work as a team member to provide comprehensive and supportive services to individuals.
  • Ability to document service provision within an online database system required.

EXPERIENCE:

  • 1-2 years’ experience collaborating with individuals with substance use and/or mental health disorders
  • Knowledge of services and supports to support individuals in reentry.

EDUCATION:

  • High School Diploma or GED
  • If not certified as a Certified Peer Specialist or Certified Recovery Specialist, will need to do within first 90 days of employment.

  • Commensurate with experience and education
  • Pay grade: 13
