Employment, education, and income are social determinants of health that are critical to the overall health and wellness of the communities we serve. Health Promotion Services seeks a mission-driven individual with experience working with young adults coming out of placement and adults who have been affected by substance use disorder to join our Workforce Development team. The Job Develop/Case Navigator is a blended position that works with the workforce development team to provide job placement, externships (to include opportunities within the healthcare sector) and wrap-around services to participants enrolled in workforce development programs and support their connections to job training, work readiness, and job search, job coaching assistance and externship placement. This individual maintains frequent contact with participants through in-person meetings, texting, phone calls, and video conferencing. This individual will also be responsible for conducting assessments, identifying and setting employment goals with participants, making appropriate referrals for supportive services, and completing periodic progress assessments, including follow-up assessments, to document participant progress. This person will also make “warm hand-offs” to employers for services and/or accompany participants for employment services appointments, as appropriate. 

Responsibilities of Job developer role:

  • Assesses participant needs and strengths with input from other team members, which include the workforce team, program and partner staff (i.e. job training and readiness).
  • Provides employment-related case management services; Administers evaluation instruments, including goal setting and closing assessments, and maintains case notes aligning with program and evaluation requirements.
  • Facilitates resume workshops and assist participants with resume preparation.
  • Conducts mock interviews with partiipants to ensure a successful inteerview process.
  • Coordinates onsite job fairs
  • Schedules virtual and or face to face meetings withemployer representatives to recruit and maintain employer partners.
  • Develops service plans that support both program and participant goals.
  • Provides warm hand-offs and linkages for participant to services to reduce barriers to employment and job retention.
  • Works with employers throughout the City to develop new leads and create a pipeline for employment and externship referrals related to securing and retaining employment with the healthcare and general employment sectors.
  • Coordinates externship process.
  • Meets participants in a variety of settings, including job preparation location, community sites and PHMC Corporate Office and community locations.
  • Participates in required trainings, conference, and program team meetings.
  • Works as part of a cross-disciplinary, cross-organization project team; has the ability to lead in peer-to-peer work environment.
  • Assists with program evaluation by administering evaluation tools, entering participant data in a timely manner into two databases.
  • Completes program reports and other written documentation as required.

Responsibilities of Case Navigator role:

  • Provides case management to enrolled participants/students to ensure successful program participation and completion.
  • Conduct assessments to assess participant/student needs in the areas of education, employability, housing status, childcare status, career and other interests, mental health status, and overall health status.
  • Creates and maintains connections to educational resources and skills-based training programs.
  • Coordinates with probation, parole officers and other referring agencies to support. participant/student attendance, progress and to coordinate needed resources.
  • Meet bi-weekly/weekly one-on-one with participant/student to monitor program progress.
  • Maintain current information on resources relevant to participant/student needs.
  • Maintain accurate files.
  • Monitor participants’/students’ daily program attendance.
  • Assist in coordinating participant/student orientation, motivational sessions, and any special programs.
  • Complete additional program duties and/or assignments as requested by supervisor.
  • Provide retention follow-up based on the stipulation of specific contract requirements for students/participants who successfully complete training and secure employment.
  • Ensure that graduates who lose their jobs are re-engaged to be connected to other employment opportunities within the appropriate contractual time.
  • Achieve and maintain job retention rates according to contractual obligations of individual programs.
  • Act as liaison between participants/students and employers.
  • Counsel participants/students about employment related issues.
  • Distribute and track program incentives.
  • Create and maintains connections to educational resources and skills-based training programs.
  • Participate in required trainings, conference, and program team meetings.
  • Complete program reports and other written documentation as required.

Skills:

  • Excellent oral and written communication skills, especially in communicating with clients.
  • Strong community engagement and positive individual development skills.
  • Ability to handle multiple responsibilities and to work independently.
  • Familiarity with Philadelphia community partners, Court adjudicated, and juvenile justice services related to custody and family court, and social service organizations.
  • Strong interpersonal skills.
  • Superior cultural competence related to race, gender, and sexual orientation.
  • Comfortable working with individuals in low-income communities.
  • Must be willing to work flexible schedule including some evening/weekend hours as requested.
  • Must successfully pass all required clearances.
  • Proficient with Microsoft Office, and web-based data collection applications.
  • Willingness to travel throughout Philadelphia.
  • Maintains integrity in handling confidential and sensitive information.

Experience:

  • At least three years direct service experience working with target population
  • Intermediate in Microsoft Office suite, Windows applications, internet and email required, experience with nonprofit databases is helpful.
  • Experience working with employers and familiarity with job market within the healthcare sector.

Education Requirement:

Bachelor’s degree in social work, workforce development, financial wellness, or related field.
