Townhouse Center for Rehabilitation and Nursing is currently looking for a LMSW with long term care experience in skilled nursing facility. Candidate must have excellent organizational skills, be able to work with a team, be a dynamic people person, have customer oriented skills. Candidate must also have experience in doing psychosocial assessment, MDS, comprehensive patient-centered care planning and be skilled at discharge planning.

The social worker will have extensive contact with residents in a sub-acute unit, families and responsible parties to ensure the unique social needs of our residents are being met. The social worker will perform all assessments on new admissions to the facility, help residents and families in understanding various processes, and will make recommendations to families and other staff members based on the unique needs of each resident. The social worker will also network with community agencies.

Requirements:

• Current Licensure as a LMSW in good standing in the state of New York
• Experience working with the ill and elderly to take care of social and behavioral needs
• Experience working with community resources
• Experience working in a skilled nursing facility
• Strong documentation, follow-up and communication skills
