Care Coordination Nurse

Job Description

Hours: Mon - Fri 9am-5pm

Department: Philadelphia FIGHT Community Health Centers

Home base: John Bell Health Center

Supervisor(s): Nurse Manager

Full-Time, 40 hours per week; Exempt

The John Bell Health Center, named in memory of AIDS activist John Bell, is a comprehensive primary care clinic operating with a team-centered, multi-disciplinary approach. It is Philadelphia FIGHT's second adult health clinic, providing care for diverse populations with varying health needs. The John Bell Health Center provides a wide array of primary care services, including contraception management, specialist and health screening referrals, and gender-affirming care. In addition to clinical care, the John Bell Health Center offers access to behavioral health and substance use disorder services, educational programs, HIV counseling and testing, and medical case management. As a Federally Qualified Health Center (FQHC), all services are available to anyone who walks through our doors, regardless of insurance status, immigration status, or ability to pay.

The ideal candidate will take a trauma-informed and harm-reduction approach to health care.

Job Responsibilities

Clinical Responsibilities (0.75 FTE)

  • Triage sick calls coming in through the Patient Access Center and regular monitoring of the shared nursing voicemail.
  • Triage sick/emergency walk-ins.
  • Assist providers in communicating lab results and providing education on recommended follow up.
  • Assist with one-on-one chronic disease management and provide patient education.
  • Identify and advocate for patients needing urgent or ongoing behavioral health services and/or substance abuse treatment via collaboration with the primary provider and/or onsite Behavioral Health Consultant.
  • Administer therapeutic injections.
  • Perform clinical tasks: i.e. EKG, oxygen setup, blood glucose monitoring, urine dipsticks and pregnancy testing as backup to clinical medical assistant.
  • Perform CLIA related tasks such as running of controls as back up to nurse manager.
  • Coordinate ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinic medical equipment (g., medical supplies and prescriptions).
  • Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, and behavioral health consultants.
  • Complete all charting and documentation as needed and required by contracted funding sources, including assessments, progress notes, and administrative documentation.
  • Complete all billing documentation as needed and required by contracted funding sources, including billing codes and required documentation.
  • Demonstrate commitment to clinical care and confidentiality


Administrative Responsibilities


  • Assist providers and nurse manager with obtaining urgent reports from diagnostic exams, specialist consults, and hospitalizations along with discussing complicated care patients with specialist offices.
  • In conjunction with medical assistants, assist providers and nurse manager, in the scheduling of imaging studies, procedures and specialty appointments, particularly those requiring prior authorization.
  • Ordering of durable medical equipment.
  • Assist medical providers in the completion of essential clinical paperwork and in the generation of letters of medical necessity.
  • Report and assist in communicating appropriate information on reportable diseases to oversight bodies, g., Public Health Department, AIDS Activities Coordination Office.
  • Coordinate ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinic medical equipment (g., medical supplies and prescriptions).
  • Assist Nurse Manager in the monitoring and reconciliation of orders in the EMR.
  • Interact closely with the other clinicians in the practice, Practice Coordinator and Administrative Director, by participating in regular meetings for purposes of addressing management issues and quality assurance.
  • Attend FIGHT case conference weekly.
  • Participate in monthly skills trainings with medical assistants and nursing team

Qualifications

Education: Bachelor's Degree in Nursing preferred

Certification:

  • Registered Nurse in Pennsylvania
  • Within 30 days of hire, complete training in Blood borne pathogens, Post exposure prophylaxis, and HIPAA regulations.
    • Prior to shipping blood specimens, complete IATA training for shipping of hazardous substances, if applicable
  • General knowledge base in science, medicine, and the running of a medical clinic. Demonstrate the ability to understand the overall purpose of clinical care and importance of support staff.
  • Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in multi-task environment. Needs to have attention to detail, dependability and flexibility.

  • Ability to continuously stand, walk or sit.
  • Ability to bend, climb stairs frequently.
  • Ability to lift up to 10 pounds infrequently.
  • Ability to squat occasionally.

Work Environment:

  • Generally works in a high-volume, clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting. May also be required to provide coverage at all Philadelphia FIGHT Community Health Centers.

Other

Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.
