PHASE-Industries, Inc. is Hiring a Direct Support Generalist Near Mora, MN

Purpose 
Provide person-centered, community inclusive support that helps a person develop and maintain essential and personally enriching life experiences. Facilitate occupational development, life enrichment activities and recreational/leisure activities. Provide training and support services including positive approaches to learning, systematic instruction, behavioral intervention, and self-care needs. Provides transportation services for individuals. 
Key Expectations

  • Person-Centered Practices - Learn and consistently perform person-centered practices: Identify what is important to and for people who receive support; implement support & facilitation strategies that, at all times, supports person-centered thinking, practices and informed choice. Learn and independently implement Person-Centered practices. Challenge co-workers and supervisors to use person-centered practices. 
  • Safety: Participate in the development of a safe and healthy workplace. Comply with safety policies and procedures and adhere to safe work practices. Comply with state motor vehicle laws and regulations. Take reasonable care to ensure your own, co-workers, and Individuals served safety and health. Report any hazards, injuries, or illnesses immediately to your supervisor. Will not place others at risk by any act or omission or interfere with safety equipment. Act as a role model by demonstrating safe work behaviors.
 
Essential Functions 
  • Direct Support Care - Act as the primary practitioner of direct support for the individuals on assigned caseload, using a person-centered approach. Direct support includes working directly with individual(s) to support them in achieving their objectives and goals, as stated in their Plan, in a manner that is consistent with the spirit of partnership, therapeutic engagement, and professional conduct. Direct support services shall be delivered consistent with the individual’s chosen communication style, language, culture, identity, choices, and preferences. Provide therapeutic support for behavioral concerns, provide de-escalation and intervention as needed. Provide therapeutic programming as recommended by OT, PT, and speech therapies. 
  • Direct support may include lifting and transferring individuals to/from wheelchairs, as well as carrying out self-care and support services (assistance in using bathroom, feeding, personal hygiene needs) as designated by the person’s preferred schedule. Assist with securing wheelchairs, transferring individuals to/from PI vehicles and public transportation vehicles.
  • Assist in promoting community connections providing domestic, community and leisure skills training and teaching to persons served, consistent with individual SP/Addendum, SMA, IAPP goals, objectives and schedules. Observes and records participant’s progress and behavior as directed. Provides input into individual’s SP/Addendum, SMA, IAPP goals, objectives and planning as directed. Assists with physical therapy and occupational therapy activities consistent with individual SP/Addendum, SMA, vocational goals. 
  • Complete job task analysis and implement community-based employment training programs, as assigned. 
  • Provide customized person-centered support that links to an individual’s informed choices and program/outcome plans. 
  • Coordinate with DSP Specialist 1 and DSP Specialist 2 to possess real-time working knowledge of participant’s plans who are pursuing various stages of the Engage, Plan, Find, Keep framework.
  • Assists with physical therapy and occupational therapy activities consistent with individual CSSP/Addendum, SMA, IAPP goals. 
  • Remote Services – On occasion, due to weather conditions, illness, or other situations causing the temporary closing of a licensed site, providing services through remote means may be necessary to ensure continuity of care with Individuals served.
  • Dispense Medication - Certified to pass medications and perform specific medicinal treatments/cares according to individual need. 
  • Support the daily operations and needs of the program. Assist in general cleaning and caretaking of the building (loading dishwasher, doing laundry, picking up work materials at the end of the day, cleaning and maintaining the refrigerator, etc.). 
  • Communication – Use Effective Communication Model in coordinating work, agreements and discussions with fellow team members. Speak clearly; listen to others; write effectively; establish relationships; act with integrity; establish trust; demonstrate a high level of respect and patience with Individuals served and coworkers. Communicates with other program staff and residential staff on a daily, or as needed basis, regarding program issues, caseload issues, staffing, and other related information.
  • Crisis Prevention and Intervention - Be knowledgeable about crisis prevention, intervention, and resolution techniques and match such techniques to circumstances and individuals. Identify the crisis, defuse the situation, evaluate, and determine an intervention strategy and contacts necessary supports. Continue to monitor crisis situations, discussing the incident with authorized staff and person(s) served, adjusting support and the environment, and complying with regulations for reporting. Use appropriate first aid/safety procedures when responding to emergencies. 
  • Lead with a service focus – Set the example for and lead others in focusing on providing excellent service for people served, their families, and all other people who support them, etc.
  • Assist individuals in accessing transportation to meet their needs. Provide transportation services as designated by the driving schedule, schedule of persons served drive and/or ride along on route, as needed or assigned. May be required to utilize personal vehicle at times in the support of persons served. 
  • Complete all required annual and assigned training on time using PHASE-Industries’ online learning management system or alternate delivery method if offered.
  • Ensure that all required documentation and paperwork is completed accurately and according to current statute and regulations. 
  • Represent PHASE-Industries and the programs/services offered within the local and surrounding community in a manner that promotes the organization and the rights & dignity of persons served and individuals with disabilities. 
  • Complies with PHASE-Industries policies and procedures. 
Job Qualifications
Minimum

  • High School education or equivalent, 18 years of age.
  • Ability to develop effective relationships and work with persons with developmental disabilities. 
  • Ability to interact well with community employers and project a positive image of PHASE-Industries in the community. 
  • Good driving record and the possession of a current Minnesota driver’s license. Willingness and the ability to use personal automobile/vehicle for agency transportation needs. 
  • Ability to be trained and certified in First Aid and CPR. 
  • Ability to pass criminal background checks requirements, vehicle insurance qualifications, and medical examination requirements (with doctor’s certification). *Health Card. 
  • Basic computer and software skills, or the ability/willingness to learn. 
Measurable Performance Benchmarks 
  • Program Participant(s) on caseload attain success in employment (vocational) as well as other person centered supports goals (measured by progress toward/accomplishing goals outlined in CSSP Addendum). 
  • DSP Team Member communicates fluently with each Program Participant on their caseload using their chosen method of communication (measured through direct observation). 
  • DSP Team Member establishes strong rapport and trust with each Program Participant on their caseload principles, clear and professional boundaries, and respectful interaction (verbal and non-verbal) (demonstrated and measured through direct observation). 
  • DSP Team Member positively contributes and interacts within their Teams, as clearly demonstrated through on-going use of effective communication and large-mindedness principles and techniques.
  • DSP Team Member learns and uses technology and software appropriately, ethically, effectively and accurately.
