Petaluma Health Center is seeking an experienced Nurse Practitioner or Physician Assistant who has adult and pediatric urgent care or emergency room experience as well as and understanding of the primary care medical home model to join our welcoming and diverse community of providers. We are considering fulltime, part-time or per diem schedules.

FULL TIME EMPLOYEE BENEFITS:
Summary: This provider will deliver patient-centered and culturally appropriate care to a diverse patient population and specifically manage patients’ acute conditions in both in-person and tele-health modalities including any urgent social and behavioral health concerns This provider will work closely with a collaborative team to ensure a proper bridge to the patient’s primary care medical home and avoidance of improper emergency room utilization.

Salary Scale: $120,000 to $173,806 per year The compensation amounts listed above do not include relocation assistance, sign-on bonus, productivity or quality/service incentives, call pay, extra shift incentives, if applicable to the position.
