Personal Health Care is Hiring a Skilled Nursing (LPN or RN) - Home Setting - Upper Darby Near Upper Darby, PA

Personal Health Care is a Medicare Certified Home Health Agency founded in 1981. We provide non-medical and skilled nursing care to clients in Bucks, Chester, Delaware, Montgomery, and Philadelphia Counties. If you would like to know more about our company, please visit www.personalhealthcare.org

We are seeking Licensed Practical Nurse or Registered Nurse to work one-on-one with our pediatric client, in-home setting, in the Upper Darby, PA area. The available shifts are Weekday and Weekends Evenings Available! Full time or Part time!

Additional Clients in surrounding area (Perkiomenville, Phoenixville, Skippack, Paoli, Wynnwood, Pottstown, etc.) Full-time or Part-time available!

Requirements for Employment

  • Current Pennsylvania Skilled Nursing License
  • Current CPR
  • Must be able to lift at least 50 pounds
  • Must pass State and Federal Criminal Background Checks
  • Must have up-to-date Physical and Two-Step PPD or Chest X-ray
  • Must be able to pass Skilled Nursing pre-employment test
  • Form of Transportation to Client's Residence

Skilled Nurse Job Responsibilities

  • Must be a caring and reliable individual who has the ability to work in a diverse atmosphere
  • Assess patient’s need for safety measures and appropriate interventions.
  • Implementation of the Nursing Care Plan as outlined by the Primary Nurse.
  • Implement medication changes according to verbal and/or written physician orders.
  • Administer prescribed medications and treatments.
  • Accurately observe and promptly report changes in the patient’s physical or mental status to the Nursing supervisor.
  • Accurately record nursing observations, activities, and treatments on the appropriate forms.
  • Ability to follow a physician's plan of care under the supervision of our Director of Nursing
  • Communicate with family members, in office staff and all supporting health professionals
