Our Personal Care Services are a key component of caring for this patient population. We support patients with complex medical conditions by placing friendly, professional caregivers in the home to assist with activities of daily living, such as bathing, grooming, meal preparation, laundry, companionship, medication reminders and others.

We are seeking a friendly, punctual, outgoing and tech savvy Registered Nurse with a positive attitude and strong work ethic to serve as our Personal Care Manager. Our ideal candidate will understand and embrace our mission of providing exceptional service to elderly individuals as well as helping them live comfortably as possible in their own homes. The right individual for this role will be responsible for scheduling care aides, communicating with and managing clients and their families, assisting with other operational responsibilities, as well as providing remote care management services to enrolled patients.

Job Duties of the Personal Care Manager include:

• Managing and engaging with staff and clients daily.
• Oversight of Independent Living Facility programs.
• Weekend on call hours (alternating weekends with other team members).
• Assist with interviews of new staff members.
• Assist with scheduling shifts, managing call outs, and scheduling new client assessments.
• Completing new client assessments and reassessments.
• Writing and updating care plans.
• Verifying visits and reviewing care notes.
• Able to complete patient care as needed.
• Communicate compassionately and professionally with clients and their family when needs arise.
• Provide remote care management services to enrolled patients, primarily via telephone conversations with the patient, family and other care providers.
• Collaborate with pharmacy on medication reminder program.

Qualifications and Requirements:

• Registered Nurse in good standing in Virginia
• 3 years' experience in personal care or home health care operations
• 3 years' experience in at least one of the following settings: case management, disease management, hospital, or physician office setting
• Electronic medical record (EMR) experience Athena EMR software preferred
• Knowledge of chronic care management (CCM) preferred
• Geriatric/Senior Adult expertise - preferred
• Strong computer skills
• Strong communication skills
• Outgoing and friendly personality.
