The ideal candidate must be a licensed social worker with experience in delivering excellent customer service and supporting key initiatives related to bleeding disorder patient adherence and retention strategies. As a Community Resource Coordinator, you will act as a community liaison at Prompt Care's bleeding disorder events, establish and maintain relationships with both existing and potential clients and patients, and work with multiple departments within Prompt Care to ensure optimal patient care. Location: Hybrid or Remote. The ideal candidate should reside in Missouri, Kansas, Nebraska OR Iowa. Requirements
Bachelor's degree or certification in social services or social work required
2-5 years social work experience in a healthcare setting preferred
Strong understanding of the regulatory social welfare programs including Medicare, Medicaid, social security, and disability
Ability to think critically and have an understanding of social services
Customer service focused
Ability to handle multiple tasks in fast paced, challenging environment
Excellent communication and people skills
Valid driver's license
Valid auto insurance
15-20% travel
Job Responsibilities, i ncluded but not limited to:
Develop relationships with bleeding disorder patients, as assigned, focusing on retention, therapy adherence, patient support, and satisfaction activities
Conducts at a minimum, quarterly and annual patient visit(s) for the bleeding disorder patient population, activities include but are not limited to: establishing rapport and building relationships, identifying resource needs, completing tasks in support of PromptCare team needs and ensuring ongoing communication with care team members to ensure support and promotion of adherence and retention strategies
Conduct telephonic outreach and engagement to bleeding disorder patients, including but not limited to: problem solving, patient advocacy, education, adherence support, and resource needs assessment and referral coordination (i.e., transportation, personal safety equipment, insurance, etc.)
Participates in patient engagement and community outreach activities in support of bleeding disorder patients/families/initiatives, as assigned
Participate in multidisciplinary teams and meetings regarding social services needs
Research available community resources which provide general and disease specific support
Develop and maintain relationships with key program contacts
Work with Patient Management to coordinate assistance with patient insurance issues
Organize and manage information regarding available support to enable patients to lead the fullest lives possible
Maintains current knowledge related to the therapies and social/community aspects of the bleeding disorder patient population
Assist in coordinating shipments and delivering medication and supplies to bleeding disorder patients, as assigned and support the provision of value-added programs for bleeding disorder patients
Perform other duties, as assigned
