PeopleCare is Hiring a Care Coordinator Near Glenwood, CO

Description

I. Position Summary:

Provides case management for an assigned caseload of clients under PeopleCare Health Services. The primary goal of Case Management is to ensure the delivery of appropriate services based on the client's care plan and that services are being provided properly and with quality by assigned care staff. In addition, case management includes coordination of care with clients SEP, family, medical personnel, and any other agencies providing services.

II. Duties/Responsibilities:

a. Case Loads: Maintain assigned evolving case load

b. 90 Day Visits/CSRs: In coordination with Agency Manager and program team members, schedule, complete, prep and document state-required 90-day visits and/or continued stay reviews (CSRs)

c. Intake Assessment: In coordination with program team members, prep and perform assessment and intake of new clients for HCBS, VA, Region 10 and Private Pay

d. Scheduling: In coordination with Agency Manager and program team members, select, schedule, and coordinate caregivers based on assessment information and care plan information for new and existing clients/consumers

e. Attend Meetings as assigned

f. Coverage Assurance: Assist Schedulers when responding to situations where scheduling conflicts, emergencies and/or “call outs” arise. Client schedule must be complete for the next business day/weekend before leaving each day. Work client shifts as required to meet regulatory requirements and company policy.

g. Client & Employee Satisfaction: Contact clients/employees via telephone call or drop in visit, on a scheduled or impromptu basis to receive feedback to ensure the service is meeting the goals outlined in the care plan. Document and respond promptly to any client or staff complaints, and review complaints with the Agency Manager.

h. Electronic Visit Verification (EVV): Ensure appropriate paperwork on an annual basis for any eligible employee is submitted on a timely basis.

i. Annual Training: To participate and provide support for Annual Training as required

j. Client Files: Keep all client files current and in compliance

k. On-Call: Accept on-call duty as assigned by the Agency Manager

l. Training and Orientation: Provide On-Boarding Caregiver check-off as needed.

m. Reporting: Enter necessary quality management and reporting responsibilities and reports as assigned by the Agency Manager

n. Regulatory Compliance: Maintain knowledge of and ensure company compliance with all applicable local, state, and federal laws and regulations concerning the operation and provision of home care service personnel including but not limited to all ongoing requirements for continuing professional education

o. Recruitment & Hiring: In conjunction with the Agency Manager and program team members, provide support efforts about the ongoing need to recruit, hire and train Caregivers, including protocols and processes

p. Surveys and Audits: To be available for all survey and audit requirements and assignments

q. Self Evaluation and self performance reviews: At a minimum on a monthly basis, journaling self-performance

III. Position Requirements:

a. Licensure/Certification/Registration: None

b. Education: High school diploma or equivalent required

c. Experience: Experience in the field of home health care is preferred but not required; experience managing a home and family or caring for disabled individuals is highly desirable; professional experience in the field of customer service

d. Special Qualifications: None

IV. Degree of Supervision Required: Involves general guidance and direction by the Agency Manager, Area Administrator, and Leadership Team. The employee will be expected to perform most job duties independently per established departmental and PeopleCare policies and procedures.

VI. Working Conditions:

  • Prolonged periods of sitting at a desk and working on a computer
  • Essential physical requirements, such as standing, kneeling, squatting, stooping, or typing
  • Physical effort/lifting, medium - up to 50 pounds
  • May be required to work weekends and nights or be on-call as a regular part of the job
