People Incorporated is Hiring a TEAM LEADER/MEDICAL LIAISON Near Rochester, NY

The Team Leader medical liaison provides clerical support to the nursing staff and direct support to the people served at the residential site. Promotes Person-Centered planning for people receiving services and their family members/guardians. Collaborates with the entire residential team including DSPs, clinicians, community agencies, and other involved in medical/nursing care as appropriate. Provide oversight and guidance to direct care staff of designated shifts. Ensure completion of required duties for each shift.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

  • Provides Direct support to person served
  • Coordinates referrals to community based medical specialty providers
  • Assists with scheduling and attending appointments and procedures with persons served as needed
  • Coordinates incoming and outgoing provider correspondence
  • Completes necessary forms as required and correspondence as needed
  • Maintains Medication Administration Certification and administers medications
  • Assists in delegated nursing care as directed by RN
  • Responsible for the organization and cleaning of the medication room.
  • Ensures sufficient stock of medications at all times
  • Communicates with clinical staff to ensure follow up to medical appointments in their clinical area.
  • Liaison with the pharmacy.
  • Checks MARS for errors and complete necessary paperwork including, use of PRN medication and ensuring documentation is complete.
  • Communicates with person served, families, and nurse regarding medical appointments and outcomes.
  • Works closely with PT/OT to understand therapy concerns and adaptive equipment needs.
  • Updates medical information in ready to go packets and individual files as needed.
  • Purges paperwork on an annual basis in conjunction with site nurse.
  • Makes notifications to day programs related to individual health needs and concerns.
  • Attends support team/clinical meetings as medical liaison.
  • Demonstrates flexibility by understanding different views. Adapts approach as situations change and accepts change in the organization and job description requirements
  • Contributes to a positive work environment by interacting in a courteous, pleasant, and professional manner. Functions as a member of an interdisciplinary team through utilization of communication skills and cooperation.
  • Maintains a safe orderly environment conducive to the health, safety and wellbeing of individuals and employees. Employing infection control practices
  • Participates in educational and training opportunities
  • Supervises and schedules the tasks/activities to ensure the services and care to each person ( i.e. shift responsibility, medical care). Provide supervision and training of direct care staff on designated shifts.
  • With support of Senior Residential Supervisor, provide staff with feedback on performance and supportive corrective action as required.
  • Ensures efficient, confidential, and comprehensive record keeping as mandated for individual and staff related activities/incidents and site maintenance.
  • Safely transports individuals to and from activities and appointments.
  • Complies with all agency policies and procedures.
Other duties as assigned.

MINIMUM QUALIFICATIONS

  • High school diploma or GED
  • 1 year of related experience preferred.
  • 18 years of age.
  • Valid Driver’s License that meets agency policy.
  • Medical competency-based training.
  • Ability to read and write.
  • Ability to handle multiple tasks simultaneously.
  • Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
  • Lifting requirement of 35 lbs.
  • Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan.
  • Ability to move, reposition, and place individuals in pieces of equipment.
  • Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.
  • CPR certification required.

SUPERVISORY RESPONSIBILITES

  • In conjunction with the Senior Residential Supervisor supervises direct care staff.
