Dover Home Health, a regional healthcare provider and true friend of adults facing terminal illnesses, seeks a service-minded, career-oriented individual to join our team as a Home Health Medical Social Worker at our St. Louis location.

As a Home Health Medical Social Worker, you will impact the lives of our patients by –

  • Providing Medical Social Services to the patient and/or family according to the Plan of Care under the direction of a physician, within the standards of professional conduct for social workers, and in compliance with the applicable laws, regulations, and agency policies.
  • Conducting a psychosocial assessment as part of the comprehensive assessment including emotional, social, financial, and environmental resources, identifying appropriate psychosocial problems, interventions, and goals, and developing an individualized plan of care in collaboration with other members of the interprofessional or interdisciplinary team, patient, family, or caregiver.
  • Observing, recording, and reporting changes in the patient’s emotional and social factors that are being affected by the patient’s illness and his/her need for care, and his/her response to treatment.
  • Assessing the patient, and/or family needs for community resources, and providing any appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient’s recovery.
  • Participating in IDG meetings, care coordination and discharge planning activities, and acting as a resource to other health care personnel in meeting patient's needs.
  • Participating in activities that promote patient safety, quality, and regulatory compliance.
  • Participating in professional development and lifelong learning activities.
  • Performing other responsibilities as assigned

To become a Dover Hospice Social Worker, you will need –

  • Has a Master of Social degree from an accredited school of social work.
  • One year of social work experience preferred.
  • Hospice or home care experience preferred.
  • Must be a licensed driver with an insured automobile in good working order, and clear driving record. Current and valid driver’s license, reliable transportation and current liability auto insurance is required.
  • The ability to communicate well, both verbally and in writing
  • Must be able to meet all physical demands of the job which includes, but is not limited to seeing, speaking, touching, walking, standing, bending, reaching overhead, crouching/kneeling, or crawling, as well as the ability to regularly lift/move up to 50lbs independently and up to 200 lbs. with assistance.
  • Ability to work well with others and take direction from management, as well as take initiative-willing to go above and beyond to ensure our patients needs are met and they are comfortable in their living environment.
  • Self-motivated to accomplish identified goals with a strong sense of accountability for results.
