Title: Substance Use Case Manager

Job Responsibilities include:

This employee will provide case management services to adult substance abuse or dually diagnosed (SA and MH) clients in the Trilogy ROH program. Assignments will be based on patient needs.

The Case Manager will work with the client in accessing, coordinating, and monitoring necessary services to assist that individual in stabilizing in the community at his/her highest potential.

The multiple roles of the Case Manager will include the following:

(A) Client identification and outreach.

(B) Individual assessment of:

(1) Basic needs such as shelter, food, clothing, entitlements, etc.

(2) Specialized needs such as substance abuse treatment services, rehabilitation, social opportunities, vocational and employment capabilities, and psychiatric services.

(3) Integrative programs such as personal skills in leisure time, church life, family communication, and other general life skills such as budgeting.

(4) Patient needs for child care and transportation in order to access services.

(C) Provide ongoing contact with community agencies necessary to develop additional resources and coordinate services, including employment.

(D) Monitoring service delivery.

(E) Client advocacy.

(F) Discharge planning.

(G) Complete KTOS.

(H) Provide educational groups.

(I) Participate in treatment team.

Requirements for this position include:

Bachelor's degree in social work, psychology, or related field within 3 months from date of hire and Certified Alcohol and Drug Counselor (CADC) credential or willingness to work toward the CADC credential. Prefer 1 years’ experience in chemical dependency field.Must attend both the Level I Case Manager and the Substance Abuse Case Management training within 6 months of employment.Must have a valid driver's license, reliable transportation, proof of automobile insurance, and must be willing to transport clients on an as-needed basis. Must pass a Criminal Record Check and provide proof of a recent (less that 1-year-old) TB skin test.

Job Type: Full-time

Pay: $16.00 - $17.00 per hour
