MDS Coordinator - Registered Nurse Assessment Coordinator (RNAC) or Licensed Practical Nurse Assessment Coordinator (LPNAC)

Requirements of the MDS Coordinator:

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN)with effective leadership and assessment skills in a long-term care setting
  • Ability to travel/ drive to assigned location a minimum of 4 days per week.
  • Must have knowledge of state and federal regulations
  • Must have at least 2 years' experience as an RNAC/LPNAC in a skilled nursing facility completing MDS
  • Proficient in PA CMI
  • Experienced in Insurance updates
  • MDS Coordinator must have Medicare billing knowledge and experience.
  • Knowledge of PPS (PDPM)
  • Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
  • COVID-19 Vaccine Required

Responsibilities of the MDS Coordinator:

  • Conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
  • Conduct and coordinate the development and completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
  • Responsible for concurrent reviews with Insurance companies/other stakeholders for continued approval of skilled care for patients
  • Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the RAI process.
  • Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
  • Participate in Facility surveys (inspections) made by authorized government agencies.
  • Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
  • Ensure that a current copy of the RAI Manual is available to persons completing portions of the MDS.
