Working for this leading academic medical center means collaboration with top clinical, technical and business professionals across all disciplines.Penn Medicine is seeking a full-time, day shift, Clinical Pharmacy Specialist, Ambulatory in Thoracic Oncology at the Hospital of the University of Pennsylvania (HUP) at the Perelman Center for Advanced Medicine (PCAM).
Summary:
Responsible for provision of pharmaceutical care with a specific focus on oral chemotherapy to patients managed in our Oncology clinics at the Penn Medicine at Perelman location. The specialist will be expected to develop a practice within the various ambulatory practice sites and liasion as appropriate with other partners. Activities include evaluate, monitor and modify drug therapy, provide recommendations to optimize patient outcomes, provide patients with new medication education, minimize safety issues and assure cost effectiveness as well as provide Drug Information for medical personnel. The pharmacist will serve as a liaison between the retail/specialty pharmacy program and the clinic to ensure patients receive prescribed medications. The pharmacist should be an effective communicator and be able to integrate themselves within the health team and current structure of the pharmacy department. Additionally, the candidate should coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the ambulatory practice setting. The pharmacist will also assist with collection and presentation of patient outcomes data and quality improvement projects in accordance with URAC Accreditation. Finally, the clinical pharmacy specialist should fulfill requirements associated with hospital and department policies and procedures, JCAHO and other regulatory agencies.

When we have the capacity, we can serve pre-9/11 veterans. We also serve family members including veteran family members and military families (National Guard, Reserves, active duty). We are not able to provide individual therapy services to active duty service members at this time. However, we can and do work with current members members of the National Guard and Reserves. Family members include parents, spouses/partners, children, siblings, caregivers, and others affected by the individual's military service.
