PHC is currently seeking a Care Coordinator to provide medical care coordination and case management services to individuals with complex medical and/or psychosocial needs who have been approved for services under the Enhanced Care Management (ECM) program. Through our partnership with Santa Clara Family Health Plan, we provide supportive services to ensure stability, self-sufficiency and overall improvement in quality of life to individuals that qualify under specific Populations of Focus (POF).Requirements
Minimum two years’ experience working with vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions and/or residing in long-term care.
Demonstrated ability to develop collaborative partnerships with staff, clients, other professionals, and community members, including individuals from diverse cultural and socio-economic backgrounds.
Valid CA driver’s license, reliable transportation, and legally required insurance.

Independently manage assigned caseload.
Provide in-person, community-based case management and care coordination services that are strengths-based, client-centered and trauma-informed.
In coordination with clinical ECM staff, complete comprehensive assessment to evaluate needs and create appropriate health action care plan.
In partnership with clients, develop mutually agreed upon care plan goals and steps to be taken toward meeting those goals.
Responsible for overall care coordination and communication between multidisciplinary providers to ensure provision of services and implementation of care plan.
Act as point of contact for patients and families involved in patients’ care team.
Assist clients to access medical, mental health and substance use treatment providers, including accompaniment to appointments as needed.
Make recommendations and referrals to governmental or community-based programs, benefits and services. Directly assist clients with linkage.
Act as an advocate for clients with social services, community-based agencies, health and behavioral health providers.
Complete documentation, data, and billing requirements in a timely manner per published procedure and timelines.
Skills Needed
Knowledge of client-centered practices to serve vulnerable populations including individuals experiencing homelessness, chronic medical conditions, and/or behavioral health concerns.
Knowledge of Medicare, Medi-Cal and managed care systems.
Knowledge of the healthcare system in Santa Clara County, particularly safety net providers and services.
Knowledge of community resources and benefit programs.
Ability to handle sensitive and confidential materials in a professional manner and in compliance with HIPAA regulations.
Ability to work on multiple projects, prioritize and meet deadlines.
Ability to communicate effectively, both orally and in writing.
Ability to work effectively and efficiently in situations with minimal supervision
