Pemi-Baker Hospice & Home Health is Hiring a Social Worker Near Plymouth, NH

Social Worker - Sign on Bonus!!!

Pemi-Baker Community Health is looking for full and part-time Home Health and Hospice Social Workers. We are looking for a compassionate BSW or MSW to work directly with our patients and their families. Must be detail oriented, well organized and have strong communications skills, both to provide understandable support and plans to clients and their families and to listen carefully to understand their issues. This position is a combination of office time as well as traveling time by car to client's homes for visitation. Will assist clients and families with gaining access to community resources.

  • Bachelor's or master's degree in social work
  • Excellent listening and communication skills
  • Ability to make psychosocial assessments and develop and implement care plans related to illness, behavioral health issues and end of life issues.
  • Palliative Care social work would include in-home or hospital consultations around advance directives, POLST, and goals of care for chronically ill and terminally ill patients.
  • Hospice bereavement support
  • Hospice volunteer coordinator duties and trainings
  • Ability to provide after-hours service as needed
  • Valid and current drivers license and insurance. Willing to drive own automobile unlimited miles and in all types of weather conditions. Mileage and travel time is paid.

Experience with homecare, hospice and electronic medical records for SW documentation is preferred but not required.

Previous hospice experience preferred, but we will train the right candidate.
