PeaceHealth is seeking a Prog Mgr Valve Center for a Full Time, 1.00 FTE, Day position. The general salary range for this job opening at PeaceHealth is $46.46 – $69.68. The actual hiring rate is dependent upon several factors, including but not limited to, the job/position responsibilities, location, terms of the applicable collective bargaining agreement, education, training, work experience, seniority, work performance, etc.

Job Summary

Responsible for the coordination of the Valve Center & Structural Heart Program. Ensures continuity of care through optimized patient assessment, clinical triage coordination, direct communication with physicians, daily patient management. Provides a continuum of patient and family education from referral to follow-up. Coordinator is the primary contact for referring physicians, patients, families, hospital and office staff, and vendors. This position oversees the OHVI Nurse Coordinators.

Essential Functions

  • Assist patient and families in understanding their diagnosis and the work-up involved to qualify for the TAVR/ Valve procedure and follow-up.
  • Develop and coordinate clinical guidelines of practice for office, OR, the ICU and the OHVI nursing unit.
  • Act as single point of contact for patients and families, including telephone triage, and the wait to be scheduled for the TAVR.
  • Follow patients through the continuum, including Pre-admission testing, inpatient admission, 30 day and one year follow up.
  • Reinforce patient education and direct patients and families to available resources and support services.
  • Ensure patients receive a treatment plan that is understandable and feasible.
  • Assess for clinical, emotional, spiritual, psychosocial and other patient/family needs.
  • Assures all Medicare requirements in National Coverage Decision for TAVR are completed.
  • Coordinate timely scheduling of tests, procedures, and appointments.
  • Develop case review presentation for case review meetings
  • Work closely with data analyst to collect TVT registry data, track outcomes and support strategic planning processes.
  • Participate in quality improvement initiatives.
  • Provides leadership for OHVI Nurse Coordinators.
  • Performs other duties as assigned.

Qualifications

Education

  • Accredited School of Nursing Required through State Board of Nursing approved program
  • Bachelor's Degree Preferred: Nursing.

Experience

  • Minimum of 5 years Required: Experience in Cardiac nursing including three years of clinical experience in a hospital environment
  • Preferred: Management and/or program development experience

Credentials

  • Required: Registered Nurse- WA
  • Required Upon Hire: Basic Life Support
  • Preferred: CVRN or CCVN certification

Working Conditions

Lifting

  • Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
  • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
  • Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
  • Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
  • Ability to move around area with frequent sitting.
  • Bending/ stooping/ squatting/ reaching/ kneeling frequently.
  • Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

Environmental Conditions

  • Exposure to biohazard, body fluids and airborne particles.
  • Must be able to complete tasks in a noisy environment.

Mental/Visual

  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
