Peace Connections, Inc is Hiring a Community Health Worker/Program Coordinator Near Newton, KS

Our vision is a thriving community sustained by peace and social justice, which we work towards through the Summer Enrichment Activities for Kids (SEAK), Harvey County Circle of Hope, and social equity programs. These relationship-based programs bridge families with community resources! Our goal is to create connections that increase access and improve the quality of life for residents throughout Harvey County.

Position Summary:

An effective Community Health Worker (CHW) helps an individual build their capacity for themselves and their families to reduce socioemotional, medical, and physical barriers by advocating, supporting, and educating individuals within Harvey County.

- Bridge gaps between individuals and larger systems through connecting resources, advocacy, and implementation of community-driven strategies.
- Collaborate with social workers and community partners to develop and implement strategies to address health inequities and social determinants of health.
- Utilize motivational interviewing techniques to encourage behavior change and educate community members on various health topics and resources available.
- Lead and coordinate monthly Laundry Love initiative.
- Co-coordinate and lead weekly Circle of Hope meetings including, but not limited to: meals, activities, speakers, and Matched Circle groups.

Key Responsibilities

Community Health Worker (35%)
o Build non-threatening and supportive relationships with community members and partners to address social determinants of health and health inequities. Provide resources and informal counseling, education, and guidance on socioemotional, medical, physical, or financial needs.
o Coordination of community outreach and events, health education, and participant recruitment.
o Coordinate and attend all CHW participant meetings.
o Work closely with individuals, staff, and other community-based organizations to help identify gaps and areas of support.
o Accurately and subjectively document meetings, track data/outcomes, and share reports with staff
o Coordinate monthly Laundry Love initiative, including meals, agencies, and funding streams.

Community Outreach (30%)
o Build and maintain community relationships by regularly attending community meetings and tabling events, disseminating and conveying education and information about programs.
o Contribute to the development and distribution of marketing and recruitment materials.
o Promote Peace Connections to individuals, churches, and organizations for awareness, financial support, and recruitment of participants, partnerships, trainers, and volunteers, in collaboration with the Event Committee and Fund Development staff.

Circle of Hope Program Coordination (35%)
o Plan and coordinate weekly Circle of Hope meetings, including scheduling presenters and leading meetings when necessary.
o Maintain weekly & monthly communication with participants and volunteers, through text/email.
o Conduct and document periodic evaluations with Circle of Hope participants and volunteers.
o Provide coaching support to Matched Circles and teaching support to the Getting Ahead Class.
o Assist with recruitment and marketing of the Circle of Hope program.
o Engage in Bridges Out of Poverty Training as well as Poverty Simulations, when necessary.
o Engage in, and support the facilitator for Volunteer Training activities.
o Provide consistent encouragement, support, check-ins, and engagement for Getting Ahead participants, before, during, and after their class up through the end of their 18-month allyship.
o Additional duties as assigned

Key Qualities
- Background in social work or related field to address social determinants of health
- Commitment to public health initiatives within the community
- Strong public speaking skills to engage with diverse audiences
- Excellent communication and relationship-building skills
- Personable, caring attitude
- Proficient computer skills (including Microsoft Office Suite and social media)
- Collaborative, bridge-builder
- Flexibility to work some nights and weekends. Hours can be flexed within the same week.

Job Type: Full-time

Pay: $37,000.00 - $41,000.00 per year
