Summary The Health Services Manager is responsible for providing oversight of the resident health care service needs.

The HSM is also responsible for the overall medication delivery system and the in- service training of the Health Services department. Essential Duties and Responsibilities include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

 Is aware and knowledgeable of the rules and regulations for Assisted Living and Memory Care in the state in which employed as well as the regulations of the state’s nursing commission.

 Participates in the move-in and ongoing assessment process per state regulations and Mosaic policy as appropriate and/or requested.  Coordinates the health care services using the RN audit tools.

 Ensures that the medication system and the resident health record are being used appropriately and adequately.  Works with the consulting pharmacy to promote an optimal medication regime for each resident. Assists with scheduling pharmacy in-services to maintain education on medication management for medication aid staff.

 Plans, communicates and coordinates with medical and/or other providers concerning the health care needs of the residents as needed and/or requested.

 Plans and executes in-services and infection control as needed per regulations and as needed or requested using a calendar.

 Supervises the limited nursing service of the Community, if provided.

 Communicates with the staff using the RN/Staff Communication Binder.

 Ensures that updated internal service plans are in place as care changes occur.

Oversee and ensure that residents weights are monitored and assessed for changes of condition needs.

 Ensures a full investigation occurs on incident reports.

 Ensures that proper charting is occurring and meeting policy.

 Ensures that PRN parameters are on the MAR and clear direction for Med Techs is available and follow up on.

 Ensures that reason for giving medication is on each medication.

 Ensure weekly documentation is in place on all skin concerns.

 Ensure the staff are using shower sheets and being checked for sores or bruising.  Ensure that all assistive devices have a current assessment.

 Ensure 90 day orders are completed.

 Verify medications orders and transcription is being processed property.

 Reviews and addresses daily missed med reports to assure resident have their required medication.

 Makes updates to evaluations and service plans as needed and per California Title 22 Regulations 30, 60, 90 days.

 Oversees and ensure completion of all clinical department quality assurance programs.

 Ensure that all care staff training, and orientation is meeting California Title 22 standards.  Assist with recruiting, hiring new staff as well as daily management of staff’s performance. (this included performance evaluation, corrective action, and terminations.)

 Performs all other duties as assigned by any member of management.

 Acts as the person in charge in the absence of the Executive Director and other managers/department heads. Minimum Qualifications and Skills

 Ability to utilize and apply basic management skills.  Provides and coordinates resident care with an awareness of the principles of assisted living.  Maintains adequate attendance, notifies supervisor if late or unable to work in accordance with personal policies, follows work schedule.  Requires minimal supervisions, is a self-starter and is flexible.  Organizes and utilizes time appropriately, sets priorities and accomplishes assigned tasks.  Displays mature behavior in attitude and speech and action and demonstrates consistently pleasant demeanor, tone of voice, supports the philosophy and approach used by the Community.  Keeps resident information confidential, respects resident’s rights, respects privacy and the right to self-determination by the residents.  Works cooperatively with the supervisors and co-workers.  Abides by established policies and procedures of the community. Looks for ways to improve community function.  Has the ability to stand for long periods of time on feet and ambulate quickly.  Is accurate, dependable, neat and thorough in assigned tasks.  Is effective in both verbal and oral communication. Education and Experience Previous experience working with the elderly an RN is desirable. Unencumbered RN license issued by the state in which employed is necessary. Maintains CPR certification. Universal Precautions Exposure to blood/bodily fluids is likely. Physical Requirements Has ability to do occasional heavy lifting (up to 40 pounds).
