You want to meet the patient and their caregivers' physical and psychosocial needs, with support of a world class interdisciplinary team consisting of primary care providers, nurses, behavioral health specialists, Patina Health Champions, and other health professionals as appropriate.

What You'll Do

• You will provide best in class care, informed by geriatric principles and rooted in respect for patients and their family's unique ability to lead their own care.
• You will provide predominantly in-home, (and as needed virtual,) primary care nursing services under the direction of primary care practitioners and in partnership with a multidisciplinary team of health champions, nurse practitioners, behavioral health specialists (BHS), and Physicians.
• You will gather patient histories, conduct physical exams, and be the in-home “eyes and ears” conducting nursing assessments that will inform Patina PCPs and shape the overall care plan and experience for patients. Often, you’ll conduct co-visits with PCPs, bringing them “into the home” via video on a tablet after you’ve conducted your nursing assessment.
• You’ll help patients and their caregivers implement care plans developed with PCPs, health champions, and BHS. You’ll help the team adapt care plans based on patient and caregiver response.
• You will work with Patina’s Electronic Medical Record (EMR) and separate care team application to efficiently and thoroughly document assessments, progress along care plans, and coordinate with your team.
• You will participate in daily team huddles and weekly interdisciplinary rounds to participate in the care of Patina patients.
• You will be an expert nurse including whole-person nursing care, integrating core geriatric principles, acute and post-acute care, shared decision making, etc.
• You will provide skilled primary care nursing services including medication administration, wound care, therapeutic treatments, and skilled teaching as ordered and indicated.
• You will aid in developing, implementing, and coordinating a system of patient care delivery that encourages communication, teamwork, positive outcomes, high quality care, patient satisfaction, and proper application of resources in order to inspire each Senior to take charge of their life and health.
• You will provide advice and emotional support to patients and their family members.
• You will participate in cross coverage with the Virtual Nurse Champion, working with a team of Health Champions to conduct virtual primary care, reinforce education, help patients meet their goals.

What's Required

• 2 years nursing experience
• A current unrestricted Registered Nurse license in Pennsylvania
• Comfortability with potentially 100% local travel across the Montgomery or Bucks County; area mileage reimbursement is included
• A current, unrestricted, and active driver’s license, as well as access to reliable transportation to visit patients with the minimum state required liability auto insurance
• Demonstrated experience navigating modern healthcare technology platforms seamlessly while providing an exceptional patient experience, ability to use different technical modalities interchangeably (eg. computer, tablet, phone) and strong learning agility for new technologies, including our care team and patient tech platforms, as well as corporate tools (e.g. Google Suite and Slack).
• Passion and demonstrated ability to build strong rapport and trusting relationship with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• BLS certification from American Heart Association
• Participation in continuing education as required by the Board of Nursing
• Yearly immunization for seasonal influenza by start date; Initial immunization and boosters against COVID-19 are highly recommended

Physical Requirements:

• Ability to lift 30 pound bag in and out of care and up and down stairs as needed
• Ability to navigate stairs and a variety of dwelling conditions and configurations
• Ability to sit, stand and kneel as needed to perform physical assessment

What Would be Great to Have:

• Home care, Geriatric, Primary Care, or Wound Care Experience
• A great relationship and understanding of the Montgomery and Bucks Counties
• Experience in start-up or fast growing and dynamic organization

Location:
Montgomery or Bucks County. This role is a predominantly home-based care role, with a blend of virtual care as needed.

At Patina, our salary bands are determined by the role and career level. The range displayed on each role posting reflects the minimum and maximum target salary (total cash compensation) for the position across all locations. Within the band, individual pay is determined based on skills, experience, capabilities, work location, required licensure and certifications. Furthermore, based on our compensation philosophy, which is grounded in hiring mission-driven and talented crew members and paying competitively while ensuring pay isn’t their sole determinant for joining or staying at Patina, we generally target the middle of the range for our total cash compensation package.

The total cash compensation band for this position is $45,000 - $55,000 equity.
