Position Summary:

  • Opportunity:Provide appropriate, patient-centered care to high-risk, medically vulnerable patients, often suffering from multiple chronic conditions, when and where they need it. This patient-centric, continuous care model delivers clinical, economic, and human benefits.
  • Mission: Patient Care Medical Group seeks to understand a patient’s physical, emotional, and social conditions to provide patient specific care with the goal of achieving an optimal level of success.
  • Solution: Facilitation of timely interventions when chronic conditions worsen and preempting avoidable emergency department visits and hospitalizations, alleviating social stressors that contribute to poor health, and comforting patients by giving them loving care and letting them know they are not alone.
  • Care Settings: Post-Acute Care (PAC), Skilled Nursing facilities, Community/Home Care Patients, Assisted Living

Essential Functions:

  • Assesses the health and nutritional needs of patients
  • Counseling patients about nutrition and healthy eating habits
  • Taking facility and patient preferences and budgets into consideration when creating meal and nutrition programs
  • Monitor and evaluate the effects of nutrition practices and plans, and adjusting if necessary
  • Talking to groups about proper nutrition and healthy eating habits.
  • Develop educational materials to promote healthy lifestyle choices and healthy food choices
  • Follow the latest research in nutrition and food science, and participating in it.
  • Develop strategic meal plans for patients with special needs.
  • Assist the patient in their counseling needs and maintaining their morale
  • Cleanliness of the workspace and using the proper utensils for food preparation
  • Communing with patients to learn more about their health goals and dietary preferences.
  • Prepare patients’ nutrition plans.
  • Track patients’ progress toward their health goals and compiling relevant information.
  • Enter patient information into the appropriate database.
  • Keep up-to-date with the latest developments in nutrition.
  • Assessment of patient’s lifestyle, nutrition, and health needs
  • Recommending patients to establish specific nutrition goals
  • Offering patients nutrition education to improve their health
  • Monitoring, tracking, and adjusting meal plans
  • Presenting facts on diet and nutrition at community events and speaking opportunities
  • Engaging in nutritional counseling to help patients develop and implement dietary plans
  • Talking to patient’s primary care physician or other health care professionals about the nutritional needs of the patient
  • Guiding patients and their families regarding nutrition, dietary plans, and diet modifications.
  • Monitoring food service operations to ensure compliance with quality, safety, and nutritional standards.
  • Coordinate and standardize recipe development, and developing new menus to support independent food service operations.
  • Developing policies that will allow food service or nutrition programs to be implemented.
  • Examining meals for compliance with prescribed diets and standard of palatability.
  • Ensuring that food is purchased in compliance with safety and health regulations.
  • Training and supervising people who prepare, and serve food.
  • Manage clinical nutrition services.
  • Coordination of diet counseling services.
  • Assist food service managers with sanitation, safety procedures, and menu development.
  • Designing, conducting, and evaluating dietary, nutrition, and epidemiological research.
  • Conducting and planning training programs in nutrition, dietetics, institutional management, and administration for medical students, staff, and the general population.
  • Making recommendations for public policy (e.g. nutrition labeling, food preparation, and nutrition standards in school programs).
  • Creating research reports and other publications that document and communicate research results..
  • Trying new food products and equipment.

Qualifications

Bachelor’s degree is required in nutrition, health, or another related field.

Additional certifications and state licenses may be required.

At least 1 year of geriatric experience
