Title: Care Coordinator
Classification: Non-Exempt
Reports to: Program Manager
$17.00/hr
Role Summary
The Care Coordinator position will provide a variety of support to the Pathways to Housing organization and patients that are served in the SCHC HOPE Medical Respite program. This position includes some field work, housekeeping work and supporting patients who need respite/recuperative care and are experiencing homelessness. The Respite program is operated in a group facility setting with 24 hour awake staff. This position works with a diverse and vulnerable population. We are looking for one early morning shift position.
Responsibilities/ Essential Functions

1. Develop rapport, communicate effectively and build relationships with patients who are placed in respite program.
2. Daily cleaning, stocking and turning over facility for the next day.
3. Conduct and maintain daily check-ins. Monitor patient daily activities and document interactions, along with patient needs in documentation system.
4. Inform Program Manager of any patient medical or hospitality issues in a timely manner.
5. Mitigate any patient issues that arise per protocol.
6. Perform basic housekeeping duties.
7. Assist with meal delivery to Scattered Site Program as needed.
8. Comply with all policies, procedures and protocols including safety protocols.
9. Assist with maintaining daily house schedule.
10. Assist patients with rule compliance.
11. Assist patients with laundry and hospitality needs as necessary.
12. All shifts require staff to stay awake and alert throughout entire shift.
13. Other duties as assigned.

Qualifications
1. Possess a valid CPR and First Aid certificate required.
2. Completion of a satisfactory background check.

1. High school diploma or equivalent required.

Work Environment
This job typically operates in a variety of conditions at the employer’s location. The employee must be able to work in a variety of weather conditions and temperatures outside, as well as within the building. The noise level may be moderate at times.
Physical Demands
This is a moderately physical position that will require bending, stooping, stretching, twisting, reaching, pulling and regularly lifting up to 40 pounds and occasionally up to 50 pounds, use of hands to grasp, move or assemble objects, wrist bending, use of equipment and materials and dealing with the outside elements. The position requires good manual dexterity (hand, hand with arm, two hands) and multi-limb coordination. This position will also require sitting, movements required for use of a computer including typing, speaking to others, and hearing.
Position Type and Expected Hours of Work
This is a part-time position, up to 29 hours. Days and hours of work include weekends and holidays. Overtime and on-call duties may be required as needed. Swing shift schedule is Wednesday-Friday 4:30pm-12:00am and Friday 3:30-12:00am (subject to change).
