Summary of Job

Pathways for People (PFP) is a human service agency providing Innovation Waiver services to individuals with Intellectual Disabilities in the community and in a Day Program setting. Our Day Program enables consumers to participate in creative/group-based programs that nurture new skills through music, art, therapeutic movement (dance, yoga, etc), technology and other activities. PFP is seeking a motivated self-starter, responsible for providing 1:1 habilitative services to consumers in accordance with the individualized plan of care. Position includes competitive wages, and health insurance options.

General Duties and Responsibilities (at a minimum)

• Establish and maintain a caring, therapeutic relationship with consumers, while ensuring their safety, health and well being
• Provide input in goal development for each consumer and implement individualized treatment plans.
• Provide an appropriate level of supervision to consumers at all times.
• Serve as a positive, healthy role model, companion and mentor to consumers.
• Defuse and de-escalate behaviors during outbursts/follow the strategies outlined in the individualized behavior plan (as appropriate)
• Act as an inspirational force, focusing on creating a safe, fun, socially stimulating environment for consumers.
• Ensure that all personal care needs are met
• Provide transportation for consumers to appointments, programs, and scheduled activities as appropriate.
• Record accurate and informative daily progress notes according to the individualized plan of care.

Qualifications

• Must possess good leadership and interpersonal skills.
• A high school diploma or equivalent required (a higher degree in a human service field, preferred but not required)
• Professional or personal experience providing direct care to individuals with disabilities preferred but not required.
• Must have a valid driver's license, and vehicle that is insured with current registration and inspections.

Job Types: Full-time, Part-time

Pay: $12.00 - $15.00 per hour
