Overview:

The Vice President of Healthcare Management and Clinical Support is responsible for the oversight, leadership, and delivery of value-based health care management programs. This position will provide clinical leadership and be the liaison internally to the Care Management Team ensuring integration of clinical supports and externally to the broader community. The role will be instrumental in external communications with stakeholders in providing a deeper understanding of the clinical needs of individuals with IDD, offering clinical expertise in the advocacy for advancements that lead to better outcomes, and the benefits of Health Home partnership toward that goal.

Responsibilities:
  • Develop and implement programs and interventions focused on improving health status of people with I/DD.
  • Use analytics and data to identify and support individuals with chronic conditions and complex needs.
  • Oversee and coordinate day-to-day operations for the HCM team and develop strategies to scale clinical and behavioral health supports to statewide care management workforce.
  • Identify mechanisms to educate, mentor, and support care management workforce in providing healthcare and behavioral health knowledge, expertise and counsel.
  • Use NYS guidance and mandated performance measures to develop care management interventions required to meet regulatory expectations; work in conjunction with the Quality and Analytics, Care Management, and all other required departments to provide appropriate information to meet defined quality program needs, identify all potential quality issues, and ensure appropriate integration of quality targets and goals.
  • Leverage clinical expertise for complex case consultation; work collaboratively with I/DD and healthcare providers to recommend treatments and strategies.
  • Ensure that clinical services within the organizations are relevant and of the highest quality possible by consistently reviewing recent literature and research of the recognized best practices, and ensuring that these practices are culturally adapted to meet the needs of all members.
Qualifications:
  • Advanced clinical degree as either a Nurse Practitioner, Physician Assistant, or MSN.
  • 5 years of relevant clinical practice experience
  • 5 years in clinical leadership positions (administration, operations, managed care, and policy making etc.)
