Partnership for Public Health is Hiring a Center Manager - Carroll County Near Tamworth, NH

The Aging and Disability Resource Center Manager (ADRC) formerly known as ServiceLink is responsible for, public education, public awareness, community and provider relations, program review and quality oversight. The Center Manager is accountable to the Human Services Manager as well as the, Deputy Director of the Partnership for Public Health, ADRC Administrator at Department of Health and Human Services, Bureau of Elderly and Adult Services. The Center Manager will provide leadership and oversight of ADRC program in Carroll County. Current services include information and referral, caregiver support, Medicare, health insurance enrollment, long-term options counseling, and services to Veterans. Responsibilities include grant writing assistance, marketing, community relations and facilitation of a regional work group dedicated to improving the health and well-being of older adults and family caregivers.

KEY RESPONSIBILITIES:

  • Work collaboratively with the Human Services Manager to establish short and long-term goals to achieve the ADRC mission;
  • Directs and evaluates program operations activities such as: logistical operations, performance measurement and reporting, management of public education and outreach, oversight of volunteer program, community provider relationship development, program review, continuous quality improvement;
  • Manages a team process within the ADRC to foster a cooperative working environment between resource center staff, on-site team members and designated community long-term services and supports providers;
  • Oversight and accountability of all day-to-day processing of ADRC programs evaluations including operational reports, quality reports, State and Federal reports to NH Department of Health and Human Services (DHHS), Bureau of Elderly and Adult Services (BEAS) by designated time frames;
  • Works with the DHHS/BEAS staff including, but not limited to: SLRC Administrator, the NH Family Caregiver Support Program Manager, SHIP/SMP Director, Adult Protective Services, and Bureau of Family Assistance to ensure that appropriate staff establish and maintain effective working relationships and coordination of the ADRC model;
  • Develops, implements, and directs public relations and communication strategies that increase public awareness and utilization of the ADRC’s various programs and services;
  • Networks with community groups to strengthen and expand the ADRC service network (faith communities, ethnic/cultural groups, health and human service providers, business, education and neighborhood groups) as well as supports activities that strengthen program recognition at the local, regional, state, and federal level; and
  • Be familiar with Cultural and Linguistic Appropriate Services Standards (CLAS) to address regional diversity and promote health equity.
  • Act as the representative of the Human Services Manager, ADRC and its mission.

EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:

  • Bachelor’s degree with 2 years’ experience in supervision preferred;
  • Obtain certification as Alliance of Information Referral Specialist (AIRS) and Person-Centered Options Counseling;
  • Experience in case management, social work, or other related human services field;
  • Experience working with Veterans, older adults, caregivers, and/or persons with disabilities,
  • Working knowledge of local, state, national resources and human service delivery system;
  • Ability to translate broad goals into achievable steps;
  • Ability to foster a team process encompassing resource center staff and designated members from outside agencies;
  • Ability to develop and oversee financial functions of the program;
  • Ability to plan, organize, direct and evaluate the work of others (including hiring, terminating, and supervising office staff);
  • Ability to train and coach staff;
  • Ability to communicate well orally and in writing;
  • Ability to collaborate among all community, regional, state, and federal groups; and
  • Good interpersonal and computer skills, openness and flexibility in working with diverse groups, enthusiasm for working collaboratively on community issues.

This position requires:

  • Minimal requirement to reach at or above shoulder level; occasional reaching below shoulder level required.
  • Ability to travel throughout the territory required.
  • Ability to work a flexible schedule, which may include night and weekends.
  • Ability to tolerate prolonged sitting and/or standing and to lift up to approximately 25 pounds to waist high level whenever necessary.
  • Mobility includes regular sitting, some standing and walking.
  • Maintain a valid driver’s license, good driving record and access to a fully insured car;
  • Criminal Records and Bureau of Elderly and Adult Services Registry record check and acceptance by agency.
