Park Avenue Health & Rehabilitation Center is seeking an Occupational Therapist to join our team, full-time. Park Avenue Health & Rehabilitation is a 200 bed skilled nursing facility with a 5-star rating from CMS and an excellent reputation within our community. The Occupational Therapist is responsible for providing rehabilitative therapy to patients upon referral for cognitive and/or ADL physical functioning disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services. This position reports to the Director of Rehabilitation Services.
