Horizon Health

Position Summary:

The LPN is responsible for the performance of nursing care assigned by and performed under the direction of the Registered Nurse. The LPN will collaborate care with Physicians, RNs, CNAs, USs, the patients and their families to develop a plan of care. He/She will communicate with ancillary departments to provide services to the patients and their families. The LPN may have contact with a variety of patients of all ages (infant, pediatric, adolescent, adult, geriatric), as well as the mentally and physically challenged.


Essential Functions:

  • Performs assessments by observations, collection, recording and reporting of objective and subjective data in an accurate and timely manner.
  • Gives input into developing nursing care plans and into adjusting care plans as changes occur.
  • Identifies and communicates unusual or abnormal observations of patient care problems to the RN/Coordinator. Notifies the RN/Coordinator Of
  • abnormal diagnostic reports and/or change of condition.
  • Administers safely and accurately all medicines within the acceptable limits of professional judgment and nursing policy.
  • Initiates IV access per physician order after completing competency per policy.
  • Observes. and regulates IV flow rates and observes and records condition of IV sites.
  • Utilizes opportunities during the delivery of patient care to teach and
  • reinforce teaching to the patient, family members, and/or significant others.
  • Reviews instructions witQpatie_n_t1-family, and/or significant others before_ transfer or discharge, and sends appropriate forms with patient.
  • Addresses customer complaints as quickly as possible by identifying steps to correct problem, communicating with coordinator/manager, implementing plan, and providing feedback to customer.
  • Maintains knowledge of job-related duties; infection control, OSHA blood
  • borne pathogens, TB/respiratory protection plan, policies and procedures.
  • Maintains patient confidentiality and avoids accessing patient information in computer except on a need-to-know basis.

The Responsibilities listed above are not all-inclusive; other activities may be required in support of the hospital’s goals and objectives.

Position Requirements:

  • Graduate from a school of licensed practical nursing ·
  • Current LPN License in Illinois
  • Knowledge of nursing policies and procedures, universal precautions and· age specific requirements.
  • Basic computer skills are required.
  • Current BLS certification
