Position Summary: 

Under the general direction of the Senior Program Manager, the Case Manager is responsible for supervising a group of Direct Service Providers who provide direct care and instruction to adult program participants in order to develop, enhance and maintain functional and daily living skills. An important aspect of the position is developing, writing and implementing the Individual Service Plans. Work includes identifying achievable goals and objectives for assigned program participants; as well as, working with the individuals to meet these goals and objectives. The Case Manager will identify, establish, and manage vocational work opportunities for participants in his/her care within the surrounding community. Supervision and direction are provided by the Senior Manager of Adult Day Training.

Essential Responsibilities 

Leadership: 

  • Provide leadership and direction for the direct service providers and ensure the overall health and safety for all individuals in program.
  • Monitor performance of all team members and provide feedback as needed.
  • Attend scheduled team meetings and actively participate in program planning.
  • Take rotating on-call responsibility to manage call offs and scheduling needs (expected to be on-call every 5th week). 
  • Other duties as assigned.

Case Management: 

  • Create structured plans and schedule for daily activities; assist direct service providers in implementation of those daily activities.
  • Responsible for documenting and maintaining comprehensive case management including the accurate and timely submission of reports such as: daily activity, progress, incident, attendance, and statistical reports as required.
  • Meet with program participant, his/her family members and significant others including representatives of community service boards and agencies, as needed to discuss the program participant's satisfaction, progress, choices, preferences, health and safety needs, etc.
  • Ensure that the coordination of vocational and non-vocational needs of the participants are being met.
  • Coordinate with a wide range of resources, both internal and external, to provide meaningful services and activities for assigned program participants.
  • Provide daily assistance to participants in areas such as physical care, behavioral support, toileting, meal preparation, feeding, mobility and other daily activities.
  • Provide crisis management and intervention as needed. Attend meetings and trainings outside of the program as necessary.
  • The ability to physically keep up with running program participants and or to lift or assist with lifting a program participant is essential.
  • Clearly communicate Parc Center for Disabilities’ mission to the outside community.
  • Act as a positive role model and clearly communicate with other team members as well as motivate and recognize accomplishments.

Requirements

Education, Training and Experience Requirements:

  • Bachelor's degree and demonstrated experience in case management / vocational services. A special skill in production, cooking, gardening, music, exercise health and wellness, or fine arts is a plus.
  • 3 years’ experience working with intellectually and developmentally disabled adults in a similar organization. 
  • At least 1 year of experience leading direct service providers. 
  • Valid Florida Drivers’ License with acceptable driving record.
  • Able to safely lift up to 50 pounds.
  • Able to stand for extended periods of time.
  • Be physically, visually, and auditorily aware of the cognitive, social, and physical needs of our participants.
  • Communicate effectively in English both verbally and in writing.
  • CPR and First Aid certifications preferred.

Knowledge, Skills & Abilities:

  • A heart for people first and foremost.
  • Have a collaborative, agile approach, with a growth mindset. The ability to quickly pivot is important.
  • Have a learning attitude and natural curiosity for how things work, with a desire to improve processes and procedures without judgement for how things were done in the past.
  • Strong background in the adults with disabilities realm, with an affinity for creativity, structure, and implementation.
  • Knowledge of Medicaid laws and ensures compliance with all such regulations.
  • Knowledge of conflict resolution and behavior management techniques.
  • Knowledge of the legal rights of program participants.
  • Ability to solve practical problems and deal with a variety of unpredictable situations where only limited standardization exists.
  • Ability to interpret instructions furnished in oral, written, diagram or schedule form.
  • Able to use independent judgment to plan, prioritize and organize a diversified workload in a fast-paced environment with real-world problem-solving skills.
  • The ability to work with confidential materials and participants’ personal information. 
  • Strong organizational skills as well as excellent communication and presentation abilities.
  • Strong analytical, written, and verbal communication, employee interaction, decision-making, case management, and organization.
  • Ability to work an irregular schedule; ability to work during office hours, evenings and weekends as needed for special program events.
  • Proficient in the Microsoft Office Suite, Therap, and other electronic tools like Zoom and Paylocity.
