About the Role:
The Social Care Navigation (SCN) team forms the backbone of Papa, playing a crucial role in enhancing our members' health outcomes by identifying and mitigating barriers to care. Our team steps in when Papa's Pals and Care Team agents unearth member social needs or health risk factors during their in-person visits and phone interactions. These insights inform our Social Care Specialists, who reach out to members, providing them with relevant resources. This approach enables SCN to build relationships with members, facilitating the detection of further needs that Papa can support.
In this role, you will be leading the SCN team and function at Papa. This includes overseeing the navigator team, directly managing SCN team managers and indirectly overseeing the navigators who work with members. Additionally, you will be responsible for setting and executing the vision of SCN. To do so, you will work cross-functionally to align on where the SCN team is going and how it fits into Papa’s long term strategy. You will then work within the team and across other groups at Papa to set the clear operating procedures and processes that deliver on company goals. You will make sure the team has the right tools and resources to do their job successfully, and you will evangelize the team internally to ensure goals are being met.
