SCHEDULE:

• 40 hours a week for 5 days
• 8:00 AM to 5:00 PM

ABOUT THE POSITION:

The Clinical Care Coordinator assists the Clinical Manager with the supervision of clinical staff to oversee productivity, compliance, and job satisfaction whilst adhering to Palmeira’s core values of preventing traumatic events for our patient population and keeping them safe and healthy in their homes. The Clinical Care Coordinator is always available to his/her staff and participates in all activities relevant to professional services rendered, including the development of qualifications and assignment of personnel.

• Responsible for the orientation of new patient care personnel. Has the ability to supervise, guide and develop nursing skills and performances.

• Coordinates with management on development of KPI’s to be updated on a regular basis to achieve goals as set forth by the QAPI program.
• Ensures and educate awareness and understanding of HHCAHPS and own role in driving HHCAHPS scores.
• Identifies and educates clinical staff on updates, changes, or edits to state, federal and company policy and procedures.
• Ensures that documentation is completed in accordance with Agency policy.
• Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed.

Requirements:

• RN License in the State of California
• Updated Credentials
  • CPR / BLS
  • Negative TB Test Result
  • Driver's License and Auto Insurance
  • Flu Shot / Signed Declination Form
  • Hep B / Signed Declination Form
• Covid-19 Vaccination / Signed Religious Exemption Form
• Immunization Records (MMR, Tdap, Varicella)
• Must have background check.

Others:

• Will be driving to patient's residences
• Must be able to lift 25 lbs
• Must be in good health and be able to fulfill the physical requirement
• Must be willing to drive all-over San Francisco
• With Home Health Care experience preferred
