Per Diem:
We offer $80.00 per visit the Full IRS of $0.67 a mile (mileage reimbursement)

An Occupational Therapist License in the state of Arizona, AZ Fingerprint Clearance Card, CPR/BLS Certification and current Negative TB Test are required for the position.
Other requirements:
- Reliable vehicle
- Driver's License and Auto Insurance
- Covid19 Vaccination Record / Signed Religious Exemption Form
- Flu Shot/signed Declination form
- Hep B test result/Signed Declination Form
- Immunizations (TDaP, MMR, Varicella)
