Job Duties:
· The Case Manager facilitates patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress
· Responsible for clinical and financial assessments of patients prior to admission
· Facilitates and actively participates in interdisciplinary planning of clinical care of patients, and assists with outcome report completion
· Provides outcome data to payers and Physicians on as needed basis
· Serves as care liaison between patient, physician, care team members, and discharge planner by coordinating, monitoring, and communicating patient’s progress
· Assists with coordination to most appropriate care level
· Initiation and implementation of plan modifications as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress
· Communicates discharge information to physician and payer with outcome data, as appropriate
Qualifications:
· RN or LVN License is required
· Case Manager experience is required
· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
· Must be knowledgeable of nursing and medical practices and procedures
