Summary of Job: The RN MDS Coordinatorassists the Directorof Nursingand the RNAssessment Coordinatorwithensuringthat documentation inthecentermeetsFederal, State,andCertificationguidelines.The RNMDSCoordinatorcoordinatestheRAIprocessassuring the timeliness, and completeness of the the MDS, CAAs, and Interdisciplinary Care Plan.

Experience: Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case- Mix,theFederalMedicare PPSprocess,andMedicaidreimbursement,asrequired. Throughunderstandingofthe Quality Indicator process.Knowledge of the OBRA regulations and Minimum Data Set.Knowledge of the care planning process.

Regulatory and Certification Responsibilities:

• AssiststhecenterinassuringadherencetoFederalandStateregulationsandcertification
• Activelyparticipatesintheregulatoryorcertificationsurveyprocessandthecorrectionofdeficiencies
• ReportstrendsfromcompletedauditstotheQualityAssuranceCommittee
• AssuresthecompletionoftheRAIProcessfrom theMDSthroughthe interdisciplinarycompletionoftheplanof care.
• InitiatesandmonitorsRAIprocesstracking,discharge/reentryandMedicaidtrackingformsthroughthe PointClickCare system.
• Followsupwithstaffwhennecessarytoassurecompliancetostandardsofdocumentation.
• Completespatientassessments,datacollection, andinterviewsstaffasnecessarytoassuregoodstandard of practice and as instructed in the current version of MDS User’s Manual
• ProvidesinterdisciplinaryscheduleforallMDSassessmentsandcareplansasrequiredbyOBRAandPPS.
• Assuresthatappropriatesignaturesareobtainedasrequired.
• EnsuresthattheInterdisciplinaryteammakesdecisions foreithercompletingornotcompletingadditionalMDS assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. (Significant Change and Significant Correction)
• AssistDirectorofNursingordesigneewithidentificationofasignificantchange,physicianordersandverbal reports to assure that the MDS and care plan are reflective of those changes.
• Coordinatesscheduling,noticeofpatientcareplanningconferences,andassurescommunicationof outcomes/problems to the responsible staff, patient, and/or responsible party.
• EnsuresallMDSinformationandcaredeliveredasoutlinedintheCarePlanissupportedbydocumentation.AssisttheAdministrator/DirectorofNursingwiththemonitoringtoensurethatacareplanisinitiatedonevery patient upon admission to the center.
• AttendsweeklyMedicareorUtilizationReviewmeetingasrequired.
• CorrectsandensurescompletionoffinalMDSandsubmits patientassessmentdatatotheappropriateStateand Federal government agencies.
• EnsurestimelysubmissionoftheMDSstotheStatewithproperfollow-uponvalidationerrors.Maintains validation records from the submission process in a systematic and orderly fashion.
