Job Overview:
We are seeking a dedicated and compassionate Navigator to join our team. As a Navigator, you will be responsible for providing support and guidance to individuals with Opiate addiction, helping them navigate through various social services and programs. Your role will involve assisting clients with accessing resources, advocating for their needs, and empowering them to achieve their goals.
Responsibilities:
-Assesses needs for MAT success
-Creates and maintains care plan
-Provides linkages to transportation, counseling services,
appointments, family education, 12 step meetings, etc.
Requirements:
-licensed individual such as an RN, LPN, or SW
-individual with a Baccalaureate degree with 2 years clinical (office) or
lived experience
-individual with an associate degree or a certified medical assistant
with 4 years clinical (office) of lived experience.
Job Types: Full-time, Part-time
Pay: $16.00 - $27.00 per hour
Expected hours: 20 – 40 per week
