BEHAVIORAL HEALTH URGENT CARE CENTER (OUTPATIENT CLINIC)

The CARE COORDINATOR , is responsible for performing assessment and reassessment of behavioral health inpatients for the purpose of performing utilization review, resource management and discharge planning. NOW OPEN – PACIFICA HOSPITAL’S BEHAVIORAL HEALTH CENTER OUTPATIENT URGENT CARE CENTER!

RESPONSIBILITIES:

• Identifies and records all information needed for initial discharge.
• Reviews medical record and determines if the intensity of services is appropriate for the severity of illness. Completes daily reviews, if necessary.
• Participates in case management meetings providing outlier reports, information on high-risk patients, and contractual account reports.
• Documents patient discharge plan and post discharge notes, updating as necessary, and makes appropriate referrals as necessary.
• Communicates with physician all admissions/discharges and concerns in order to stay within length of stay guidelines and reduce avoidable days. Interacts with providers to clarify/revise treatment plan.
• Communicates with physician regarding discharge planning needs of the patient. Assist the physician in facilitating post hospital care.
• Initiates ongoing communication with patient, patient’s family and/or caretaker to assess discharge needs. Communicates discharge plan to patient, patient’s family and/or caretaker.
• Treats patient, patient’s family and/or caretaker with respect and dignity.
• Reports and participates in authorization for outside provider services. Obtains authorization from the treating physician as necessary.
• Coordinates discharge plan with other disciplines of the health team to ensure the continuum of care.
• Collaborates with Patient Financial Services staff
• Assists all departments in the development of meaningful criteria to evaluate the quality and appropriateness of the care provided
• Develops and recommends corrective action plans to be implemented when indicator measurement is below pre-determined thresholds or above benchmarks
• Develops and standardizes reports, questionnaires, and conducts comparative studies, data analysis and interpretation
• Acts as liaison for all departments and services as they relate to Performance Improvement

EDUCATION/TRAINING/EXPERIENCE:

• One (1) year experience in an acute care facility, with Behavioral Health experience
• Degree in Social Work (BSW, MSW), Licensed Vocational Nurse, Registered Nurse or related field
• Basic Life Support (BLS)
• Excellent communication (written / verbal) skills
• Excellent organizational skills
• Excellent critical thinking and analytical skills
