Pacific Clinics serves children, transitional age youth, families, adults, and older adults. About Our Program 

Program Type: First Five Program
Population Served: 0-5

The Pacific Clinics First 5 program is contracted with Santa Clara County Mental Health. First 5 provides services to children 0 to 5 and their families or support system. Services include home visitation and therapeutic services. 

JOB SUMMARY

Scheduled Hours per Day: Mon-Friday 9am-5 pm

  • Works within the vision, mission, and philosophy of the agency.
  • Provides assessment and related developmental and psychosocial services to youth and their families by engaging and partnering with parents/caregivers so that they can meet the developmental needs of a youth.
  • This position will be part of a multi-disciplinary team.

RESPONSIBILITIES AND DUTIES

  1. Provides direct support to youth aged six (6) and under and families in a variety of settings.
  2. Conducts assessments of youth aged six (6) and under.
  3. Participates in multi-disciplinary team to develop initial treatment plans and monitors
  4. progress toward goals.
  5. Provides support counseling and crisis intervention for youth and families.
  6. Works with the family in the development and achievement of their case plan goals.
  7. Partners with family to identify and access community-based resources and strategies
  8. as they pertain to the five protective factors including but not limited to nutritional,
  9. medical, dental, drugs and alcohol abuse support services.
  10. Partners with caregivers in the development of agreed upon skills associated with
  11. acquisition of concrete needs, positive behavior management and parenting strategies,
  12. self-care and coping skills, safety planning, and community interactions.
  13. Mentors and supports families to effectively identify and access services to address
  14. and resolve emergency situations.
  15. Works with the family to develop reunification strategies and associated safety plans.
  16. Participates in staff meetings, case conferences, training, family meetings, and
  17. professional consultation as necessary.
  18. Follows states laws regarding confidential information in verbal and written
  19. communication.
  20. Works collaboratively with members of the network partners and their services.
  21. Prepares reports and recordings in a timely manner.
  22. Actively participates in team meeting to effectively share issues, opportunities and successes to help ensure the entire multi-disciplinary team is “on the same page” and can perform at the highest possible level.
  23. Maintains record of all family contact and services used by youth, completes timely and accurate documentation.
  24. Follows up with families after services have been rendered to determine if service was successful or additional service is needed.
  25. Coordinates, disseminates, and collects outcome evaluation instruments.

OTHER DUTIES AND RESPONSIBILITIES

  1. Performs other duties as directed by the Clinical Program Manager.
  2. Performs other responsibilities, as assigned, to support department/business needs.

 CORE ABILITIES

Ability to: Lead discussion, conceptualize well, and communicate effectively. Advocate for families served within the larger community and other macro systems. Mobilize and educate families about formal and informal resources to support families.

QUALIFICATIONS

MINIMUM EDUCATION AND/OR EXPERIENCE

  • Bachelor's Degree (BA/ BS) in Child Development and Developmental Interventions, Social Work or related field required.

Preferred Qualifications 

  • At least three (3) years’ experience working with youth age 0-5 and their families preferred – each year of education beyond a Bachelor's Degree (BA/ BS) is acceptable as experience required.

 PREFERRED QUALIFICATIONS

  • Master’s Degree (MA/ MS) in social work, child development, or related field preferred.
  • At least one (1) year of experience working with youth aged 0-5 and their families preferred.
  • Knowledge of early childhood development and behavior, including foundations of infant-family early childhood mental health preferred.
