Social Work Manager

What is a PACE Program?

The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Staffing ratios are much lower, time spent with each participant is much greater, and team collaboration occurs much more frequently than in other care settings. PACE is a very fulfilling health care delivery model for its employees.

Social Work Manager responsibilities include but are not limited to:

· Creates treatment plans that reflect participant values, goals, and health. Applies feedback from the Interdisciplinary Team regarding participant treatment plans.

· Conduct or participate in home visits as appropriate to ensure mental health needs of the PACE participant community are compliant and being met.

· Facilitate regular team meetings with the Social Work department. Document and update direct report as appropriate.

· Serve as a liaison for department specific staff by updating leadership as needs or concerns arise.

· Demonstrate and foster positive relationships amongst staff, with a focus on team building.

· Performs as a role model demonstrating PACE values within the organization and community.

· Participate in department specific policy development and provide suggestions for updates as needed and appropriate.

· Ability to interpret PACE policy and procedures.

· Effectively and confidently relay direction to department specific staff as necessary.

Skills:

· Possess strong analytical and critical thinking skills.

· Possess effective verbal and written communication skills, organizational skills, and the ability to exercise appropriate judgement.

· Possess strong listening skills and ability to assess situations with professionalism and courtesy.

· Ability to function independently and have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.

· Demonstrates proficient computer skills using Microsoft Office, Excel, and other business software programs.

Education and/or Experience:

· Master’s degree in Social Work. Must be licensed in the State of Michigan.

· Preferred fully licensed MSW or willingness to obtain full MSW licensure within 6-12 months of hire.

· Minimum two (2) years’ experience in health-related field, and supervisory/management/Leadership role.

· Preferred experience working with an age advanced population.

· Current and unencumbered MI driver’s license.
