The compensation for this position is provided on a monthly basis as it is grant-funded through the VA of Memphis.

```Duties```

- Provide case management services to veterans
- Conduct comprehensive assessments to identify client needs and develop appropriate service plans
- Collaborate with clients to set goals and develop strategies for achieving them
- Coordinate and monitor the delivery of services, ensuring that clients receive the necessary support and resources
- Advocate for clients and connect them to community resources and social services
- Conduct regular follow-up meetings with clients to assess progress and make necessary adjustments to service plans
- Maintain accurate and up-to-date case records and documentation

Key Responsibilities:

Coordinate, monitor, and evaluate the delivery of wrap-around supportive services to individuals served, ensuring their comprehensive needs are met effectively.

Conduct thorough assessments to identify the needs and strengths of veterans, and develop comprehensive plans aimed at achieving residential stability and fostering independence.

Prioritize patient safety with a sense of urgency while providing exceptional customer service to program participants.

Ensure the overall well-being of veterans by fostering a positive and supportive environment within the program.

Provide outreach support, engaging with individuals to connect them with needed resources and services.

Qualifications:

Bachelor’s Degree or Master’s Degree preferred in Social Work, Psychology, Counseling, or related field.

Relevant experience in case management, outreach coordination, or social services preferred.

Strong organizational and assessment skills.

Exceptional communication and interpersonal abilities.

Ability to prioritize tasks and work effectively in a fast-paced environment.

Work Schedule and Compensation: Up to 40 hours per week

Job Type: Full-time

Pay: $19.00 per hour

Expected hours: 30 per week

Patient demographics:

  • Adults
  • Older adults

Schedule:

  • 8 hour shift
  • Monday to Friday
