MDS Coordinator- SIGN ON BONUS
Ozark Riverview Manor is a not for profit, skilled nursing facility. We are looking for motivated and caring individuals to join our team. We have a full time position for an RN to perform the role of MDS nurse, prior experience is not required we will send to training for this job. Must be willing to take weekend call at times and fill in on the floor as needed as a charge nurse when the need arises to assist the nursing team.
GENERAL SUMMARY OF DUTIES:
Responsible for conducting and coordinating the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS) and the electronic transmission of data to the state of Missouri. The coordination of appropriate participating health professionals for the purpose of conducting initial and periodical comprehensive, accurate assessments of each resident to plan care that allows the resident to reach his/her highest possible of physical and mental and psychosocial functioning.
Qualifications:
Comprehensive knowledge of modern nursing techniques, routines and methods as applied to skilled care and of state and federal resident care regulations.
Ability to make independent decisions on a regular basis.
Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
Ability to encourage a proper professional attitude among the members of the nursing staff.
Ability to prepare accurate and detailed records and reports.
Ability to effectively present information to customers, clients, and other employees of the organization.
Experience with personal computers.
Essential Duties and Responsibilities:
RAI (Resident Assessment Instrument) preparation: schedules completion of RAI following federal and state guidelines; assists in completion of RAI as needed; screens each MDS for accuracy before finalizing; transmit RAI data to the state of Missouri; place copy of completed and signed MDS in resident record; completes transmit discharge tracking form; place copy of discharge tracking form in resident record; and review facility reports and correct if errors in possible.
Sends PDPM/RUGS scores to the state of Missouri.
Completion of CAA’s (Care Area Assessment) and summaries.
Evaluates all patients to establish appropriateness of admission and continued stay. Makes recommendations for care needs, notifies/communicates with physicians concerning patient care needs.
Works collaboratively with Social Services Department and other departments as needed to achieve discharge plan.
Monitors procedures/tests ordered for appropriateness/necessity.
Oversees complex continuity of care planning including facilitating care plan meetings as needed.
